Description
Overview of the medicine
Glims 2 mg Tablet contains Glimepiride, an oral antidiabetic drug used to lower blood glucose levels in patients with type 2 diabetes mellitus.
Uses & Indications
Dosage
Adults
Initial dose is 1-2 mg once daily, administered with breakfast or the first main meal. The dose may be increased incrementally based on blood glucose response, up to a maximum of 8 mg per day.
Elderly
Start with a lower initial dose (e.g., 1 mg once daily) and titrate carefully, as elderly patients may be more sensitive to the hypoglycemic effects.
Renal_impairment
For patients with mild to moderate renal impairment, a reduced initial dose of 1 mg once daily is recommended, with careful titration and monitoring. Not recommended for severe impairment.
How to Take
Take Glims 2 mg tablet orally once daily with breakfast or the first main meal of the day. Swallow the tablet whole with water; do not crush or chew.
Mechanism of Action
Glimepiride stimulates insulin release from pancreatic beta cells and may also enhance peripheral insulin sensitivity.
Pharmacokinetics
Onset
Onset of action typically occurs within 2-3 hours.
Excretion
Approximately 60% of the dose is excreted via urine and 40% via feces, as metabolites.
Half life
The plasma half-life is approximately 5-9 hours.
Absorption
Well absorbed from the gastrointestinal tract, with bioavailability approaching 100%. Food intake does not significantly affect absorption.
Metabolism
Extensively metabolized in the liver by CYP2C9 to two main inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to glimepiride or other sulfonylureas or sulfonamides.
- Type 1 diabetes mellitus, diabetic ketoacidosis (with or without coma).
- Severe renal or hepatic impairment.
- Pregnancy and breastfeeding.
Drug Interactions
Corticosteroids, Diuretics, Thyroxine, Oral contraceptives
May reduce the hypoglycemic effect of glimepiride, leading to increased blood glucose levels.
Beta-blockers, NSAIDs, Salicylates, Sulfonamides, ACE inhibitors
May potentiate the hypoglycemic effect of glimepiride, increasing the risk of hypoglycemia.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose can lead to severe hypoglycemia. Management includes oral glucose administration for conscious patients, or intravenous glucose/glucagon for unconscious patients. Close monitoring is essential.
Pregnancy & Lactation
Glimepiride is contraindicated during pregnancy and breastfeeding. Insulin is generally preferred for glycemic control in pregnant women with diabetes.

Lab Monitoring
- Regular monitoring of blood glucose levels (fasting, postprandial) and HbA1c.
- Periodic assessment of renal function (creatinine, GFR) and liver function tests (ALT, AST).
Doctor Notes
- Emphasize patient education regarding hypoglycemia symptoms, prevention, and management.
- Monitor renal and hepatic function periodically, especially in vulnerable populations.
- Review concomitant medications for potential drug interactions.
Patient Guidelines
- Always take Glims exactly as prescribed by your doctor.
- Learn to recognize the symptoms of hypoglycemia (e.g., sweating, trembling, confusion) and carry a source of sugar.
- Maintain a consistent meal schedule and do not skip meals.
Missed Dose Advice
If you miss a dose, take it as soon as you remember, unless it is almost time for your next dose. Do not take a double dose to make up for a missed one.
Driving Precautions
Patients should be cautious while driving or operating machinery, especially at the beginning of treatment or if hypoglycemia occurs, as glimepiride can impair concentration and reaction time.
