Description
Overview of the medicine
Glipita 100 mg Tablet contains Sitagliptin, an oral antidiabetic drug used to improve glycemic control in adults with type 2 diabetes mellitus. It helps the body produce more insulin when blood sugar is high and reduces the amount of sugar made by the liver.
Uses & Indications
Dosage
Adults
The recommended dose is 100 mg once daily, with or without food.
Elderly
No dose adjustment required based on age alone. Dose adjustment may be necessary if renal impairment is present.
Renal_impairment
For mild renal impairment (CrCl ≥50 mL/min), no dose adjustment. For moderate (CrCl ≥30 to <50 mL/min), 50 mg once daily. For severe (CrCl <30 mL/min) or ESRD requiring dialysis, 25 mg once daily.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Sitagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that enhances the levels of active incretin hormones (GLP-1 and GIP). These hormones increase insulin synthesis and release from pancreatic beta cells and decrease glucagon secretion from pancreatic alpha cells in a glucose-dependent manner.
Pharmacokinetics
Onset
Within hours of administration, sustained glucose-lowering effect seen over time.
Excretion
Primarily excreted unchanged in urine (approx. 79%), with minor fecal excretion (approx. 13%).
Half life
Approximately 12.4 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations (Tmax) reached in 1-4 hours.
Metabolism
Minimally metabolized, primarily by CYP3A4 and to a lesser extent by CYP2C8.
Side Effects
Contraindications
- Hypersensitivity to Sitagliptin or any component of the tablet.
- History of serious hypersensitivity reaction (e.g., anaphylaxis, angioedema) to any DPP-4 inhibitor.
Drug Interactions
Digoxin
May increase digoxin levels; monitor digoxin levels if co-administered.
Metformin, Sulfonylureas, Thiazolidinediones, Insulin
Increased risk of hypoglycemia when combined with these agents; a lower dose of the co-administered agent may be required.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In clinical trials, single doses up to 800 mg and multiple doses up to 600 mg/day for 10 days were well tolerated. Management of overdose should involve standard supportive measures. Sitagliptin is modestly dialyzable.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of teratogenicity. Limited data in pregnant women. Use during pregnancy only if clearly needed. It is not known if Sitagliptin is excreted in human milk; caution should be exercised when administered to a nursing woman.

Clinical Trials
Numerous clinical trials have demonstrated the efficacy and safety of Sitagliptin in improving glycemic control in patients with type 2 diabetes, both as monotherapy and in combination with other antidiabetic agents. Studies include long-term cardiovascular outcome trials (e.g., TECOS trial) showing cardiovascular safety.
Lab Monitoring
- Glycated Hemoglobin (HbA1c) levels
- Blood glucose levels (fasting and postprandial)
- Renal function (e.g., eGFR, creatinine) periodically, especially in patients with existing renal impairment.
Doctor Notes
- Counsel patients on lifestyle modifications (diet, exercise) as these are fundamental for diabetes management.
- Educate patients on symptoms of hypoglycemia, especially if on combination therapy.
- Monitor renal function periodically.
- Advise patients to seek immediate medical attention if they experience severe, persistent abdominal pain, which could be a sign of pancreatitis.
Patient Guidelines
- Take as directed by your doctor, usually once daily.
- Do not stop taking this medicine without consulting your doctor.
- Inform your doctor about all other medications you are taking.
- Report any unusual symptoms, especially severe abdominal pain (possible pancreatitis).
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Sitagliptin itself is unlikely to affect the ability to drive or operate machinery. However, if used in combination with other antidiabetic agents that can cause hypoglycemia (e.g., sulfonylureas, insulin), patients should be warned about the risk of hypoglycemia, which might impair driving ability.
Lifestyle Advice
- Maintain a healthy diet recommended for type 2 diabetes.
- Engage in regular physical activity.
- Monitor blood sugar levels regularly as advised by your doctor.
