Description
Overview of the medicine
Gatifloxacin is a broad-spectrum fluoroquinolone antibiotic used to treat bacterial infections in various parts of the body.
Uses & Indications
Dosage
Adults
Typically 200-400 mg once daily, depending on the infection type and severity. For 250mg, usually 250 mg once daily for 5-10 days.
Elderly
No specific dosage adjustment based on age alone; monitor renal function.
Renal_impairment
For creatinine clearance < 40 ml/min, dose adjustment is required (e.g., 200 mg every 24 hours).
How to Take
Gloxen-250 mg tablet should be taken orally with or without food. Do not take within 2 hours of antacids (containing aluminum, magnesium, or sucralfate), iron supplements, or multivitamins containing zinc, as they can significantly reduce absorption.
Mechanism of Action
Gatifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Varies by infection type, typically within hours to days for clinical improvement.
Excretion
Primarily excreted unchanged in urine (about 70% of a dose), with a smaller portion via feces.
Half life
Approximately 7-14 hours, enabling once-daily dosing.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations (Cmax) reached within 1-2 hours. Absolute bioavailability is approximately 96%.
Metabolism
Minimally metabolized by the liver; less than 1% of the dose is metabolized.
Side Effects
Contraindications
- Hypersensitivity to gatifloxacin or other fluoroquinolones
- History of tendinitis or tendon rupture associated with fluoroquinolone use
- History of myasthenia gravis
Drug Interactions
Warfarin
Increased anticoagulant effect; monitor INR closely.
Corticosteroids
Increased risk of tendon rupture when co-administered.
Antacids, iron, zinc-containing products
Reduced absorption of gatifloxacin. Administer gatifloxacin at least 2 hours before or 4 hours after these agents.
Drugs that prolong QT interval (e.g., Class IA and III antiarrhythmics, tricyclic antidepressants, macrolides)
Increased risk of QT prolongation and arrhythmias.
Storage
Store in a cool, dry place, below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. Gastric lavage may be considered. Hemodialysis and peritoneal dialysis are minimally effective in removing gatifloxacin from the body.
Pregnancy & Lactation
Pregnancy Category C. Gatifloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted into breast milk; therefore, a decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.

Clinical Trials
Extensive clinical trials have demonstrated gatifloxacin's efficacy and safety for its approved indications, although systemic use has been restricted or withdrawn in some regions due to safety concerns (e.g., dysglycemia).
Lab Monitoring
- Renal function, especially in patients with pre-existing renal impairment or elderly patients.
- Blood glucose levels, particularly in diabetic patients or those on concomitant glucose-lowering drugs.
- Liver function tests if abnormalities develop or in patients with pre-existing hepatic impairment.
Doctor Notes
- Emphasize patient education regarding the black box warnings, particularly tendinopathy, peripheral neuropathy, and dysglycemia.
- Carefully assess patient history for risk factors such as age, concomitant corticosteroid use, diabetes, and pre-existing cardiac conditions before prescribing.
- Advise patients to seek immediate medical attention if symptoms of serious adverse reactions occur.
Patient Guidelines
- Complete the full course of treatment as prescribed, even if symptoms improve, to prevent antibiotic resistance.
- Avoid excessive sunlight or artificial UV light, as gatifloxacin can cause photosensitivity. Use sunscreen and wear protective clothing.
- Report any signs of tendon pain, swelling, or rupture (e.g., in the Achilles tendon) immediately to your doctor.
- Monitor blood sugar levels closely, especially if you are diabetic.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Gatifloxacin may cause dizziness, lightheadedness, or visual disturbances, which could impair your ability to drive or operate machinery. Exercise caution until you know how the medication affects you.
Lifestyle Advice
- Maintain adequate hydration by drinking plenty of fluids.
- Avoid alcohol consumption if it exacerbates gastrointestinal side effects.
- Practice good hygiene to prevent recurrence of infections.
