Overview of the medicine
Gloxen 500 mg tablet contains Levofloxacin, a fluoroquinolone antibiotic used to treat a wide range of bacterial infections including respiratory tract infections, skin infections, urinary tract infections, and community-acquired pneumonia.
Uses & Indications
Dosage
Adults
Typically 500 mg once daily for 7-14 days depending on the infection.
Elderly
No specific dose adjustment required based solely on age, but renal function should be assessed.
Renal_impairment
Dosage adjustment is necessary for patients with creatinine clearance < 50 mL/min. For CrCl 20-49 mL/min, initial dose 500 mg then 250 mg every 24 hours. For CrCl 10-19 mL/min, initial dose 500 mg then 250 mg every 48 hours.
How to Take
Take orally with or without food. Ensure adequate hydration.
Mechanism of Action
Levofloxacin acts by inhibiting bacterial DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.

Typically within 1-2 hours.
Excretion
Primarily excreted unchanged in the urine (about 87% of an oral dose within 48 hours).
Half life
Approximately 6-8 hours.
Absorption
Rapid and nearly complete absorption after oral administration. Peak plasma concentrations are usually attained 1-2 hours after dosing. Bioavailability is approximately 99%.
Metabolism
Minimally metabolized in the liver; approximately 5% of the dose is metabolized.

Contraindications
- Hypersensitivity to levofloxacin or other fluoroquinolones
- History of tendinitis or tendon rupture associated with fluoroquinolone use
- Epilepsy
- Children and growing adolescents
Drug Interactions
NSAIDs
May increase the risk of CNS stimulation and convulsions.
Warfarin
May enhance anticoagulant effect, requiring INR monitoring.
Corticosteroids
Increased risk of tendon rupture when co-administered.
Antacids containing magnesium or aluminum, sucralfate, iron supplements, multivitamins containing zinc
Reduce absorption of levofloxacin; separate administration by at least 2 hours.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include CNS effects (confusion, convulsions, tremors), crystalluria, acute renal failure. Treatment is symptomatic and supportive, including gastric lavage and adequate hydration.
Pregnancy & Lactation
Not recommended during pregnancy due to potential for cartilage damage in the fetus. Excreted into breast milk, advise against use during lactation.
