Overview of the medicine
Gluconor 1 mg Tablet is an oral antidiabetic medicine used to lower blood glucose levels in patients with type 2 diabetes mellitus. It belongs to the sulfonylurea class of drugs.
Uses & Indications
Dosage
Adults
Initial dose is 1-2 mg once daily, taken with breakfast or first main meal. Max dose: 8 mg/day.
Elderly
Start with 1 mg once daily and titrate carefully based on glucose monitoring.
Renal_impairment
Initial dose of 1 mg once daily; dosage adjustments required based on renal function and glucose monitoring. Avoid in severe renal impairment.
How to Take
Take orally, once daily with breakfast or the first main meal. Swallow whole with water. Do not chew or crush.
Mechanism of Action
Glimepiride stimulates insulin release from the pancreatic beta cells by binding to the sulfonylurea receptor (SUR1) on the cell surface, leading to closure of ATP-sensitive potassium channels and subsequent depolarization, which opens voltage-gated calcium channels and triggers insulin secretion.

Approximately 60% of the dose is excreted in the urine and 40% in the feces. Both metabolites are non-active.

Metabolism
Completely metabolized in the liver by CYP2C9 to two major metabolites, a cyclohexylhydroxy methyl derivative and a carboxyl derivative.

- Hypersensitivity to glimepiride or other sulfonylureas or sulfonamides.

Glimepiride may potentiate the effect of warfarin.
