Description
Overview of the medicine
Hemorise 100 mg Injection is an intravenous iron preparation used to treat iron deficiency anemia in patients where oral iron is ineffective or cannot be tolerated. It provides elemental iron to the body, essential for hemoglobin formation and red blood cell production.
Uses & Indications
Dosage
Adults
The usual cumulative dose is 1000 mg of elemental iron, administered as 100 mg (5 mL) to 200 mg (10 mL) slow intravenous injection or infusion, 1 to 3 times per week until the cumulative dose is achieved. Individual doses should not exceed 200 mg given 3 times per week.
Elderly
No specific dose adjustments needed for elderly patients, but caution is advised due to potential comorbidities.
Renal_impairment
Frequently used in chronic kidney disease; dosage depends on iron status and erythropoietin therapy. For hemodialysis patients, 100 mg intravenously after each dialysis session, up to 1000 mg cumulative dose.
How to Take
Hemorise 100 mg Injection should be administered slowly by intravenous injection or by intravenous infusion after diluting in 0.9% NaCl. It should not be administered intramuscularly. Patients should be observed for signs and symptoms of hypersensitivity reactions for at least 30 minutes following administration.
Mechanism of Action
Iron sucrose complex dissociates into iron and sucrose. The released iron is then transported by transferrin to the bone marrow for erythropoiesis (red blood cell production), where it is incorporated into hemoglobin. It also replenishes iron stores in the body.
Pharmacokinetics
Onset
Serum iron levels increase rapidly; hemoglobin response typically seen within weeks.
Excretion
Minimal renal excretion of intact iron sucrose. Iron is mostly reutilized or stored. Sucrose is predominantly excreted renally.
Half life
The elimination half-life of iron from the complex is approximately 6 hours.
Absorption
Rapidly distributed after intravenous administration. Iron component is delivered to the reticuloendothelial system.
Metabolism
Iron sucrose dissociates into iron and sucrose. Iron is primarily utilized in erythropoiesis or stored as ferritin/hemosiderin. Sucrose is largely eliminated via urine or metabolized.
Side Effects
Contraindications
- Known hypersensitivity to iron sucrose or to any of its excipients.
- Anemia not attributed to iron deficiency (e.g., hemolytic anemia).
- Iron overload (e.g., hemochromatosis, hemosiderosis) or disturbances in iron utilization.
- First trimester of pregnancy.
Drug Interactions
ACE Inhibitors
May increase the risk of hypersensitivity reactions, although clinical significance is not well established.
Oral Iron Preparations
Concomitant administration of oral iron preparations may lead to reduced absorption of oral iron. Oral iron therapy should be started at least 5 days after the last Hemorise injection.
Storage
Store below 30°C. Do not freeze. Protect from light. Keep out of reach of children.
Overdose
Overdose may lead to acute iron overload, which can manifest as hypotension, circulatory collapse, and potentially liver damage. Treatment involves supportive measures and iron chelating agents (e.g., deferoxamine) if necessary.
Pregnancy & Lactation
Pregnancy Category B. Hemorise should be used during pregnancy only if clearly needed. Iron sucrose is excreted in breast milk; however, adverse effects on the breastfed infant are unlikely. Consult a doctor.

YesClinical Trials
Clinical trials have demonstrated the efficacy and safety of iron sucrose in treating iron deficiency anemia in various patient populations, including those with chronic kidney disease, inflammatory bowel disease, and post-bariatric surgery. Studies show significant increases in hemoglobin and ferritin levels.
Lab Monitoring
- Hemoglobin and hematocrit levels
- Serum ferritin (to assess iron stores)
- Transferrin saturation (TSAT) (to assess iron availability)
- Regular monitoring of vital signs during and after administration.
Doctor Notes
- Prior to administration, ensure patient's iron status (Hb, ferritin, TSAT) is assessed.
- Careful monitoring for hypersensitivity reactions is crucial during and immediately after infusion.
- Educate patients on potential side effects and when to seek immediate medical attention.
Patient Guidelines
- Inform your healthcare provider about all your medical conditions and medications you are taking.
- Report any adverse reactions, especially signs of an allergic reaction (e.g., rash, itching, swelling, difficulty breathing) immediately.
- Do not take oral iron preparations without consulting your doctor while on Hemorise injection.
- Complete the full course of treatment as prescribed by your doctor.
Missed Dose Advice
Since Hemorise is administered by a healthcare professional, a missed dose is unlikely. If a scheduled dose is missed, contact your doctor or nurse to reschedule the administration.
Driving Precautions
Hemorise 100 mg Injection may cause dizziness or lightheadedness. Patients should be advised not to drive or operate machinery if they experience these symptoms.
Lifestyle Advice
- Maintain a balanced diet rich in iron to help sustain iron levels.
- Follow your doctor's recommendations for managing your underlying condition causing iron deficiency.
