Description
Overview of the medicine
Hemorise 500 mg Injection contains Tranexamic Acid, an antifibrinolytic agent used to prevent or reduce excessive bleeding in various medical conditions, including surgical procedures, heavy menstrual bleeding, and certain types of hemorrhage.
Uses & Indications
Dosage
Adults
Typically 500 mg to 1 g (1 to 2 ampoules) by slow intravenous injection (1 mL/minute) two to three times daily. Dose adjusted based on condition and weight.
Elderly
Dose adjustments may be necessary in elderly patients with impaired renal function.
Renal_impairment
Dosage must be reduced according to serum creatinine levels to avoid accumulation. E.g., CrCl 30-60 mL/min: 10 mg/kg twice daily; CrCl 10-30 mL/min: 10 mg/kg once daily.
How to Take
Administer slowly by intravenous injection (not more than 1 mL per minute) to avoid hypotension. Can be diluted with common intravenous solutions like saline or dextrose.
Mechanism of Action
Tranexamic acid competitively inhibits the activation of plasminogen to plasmin, a molecule responsible for the breakdown of fibrin clots. By inhibiting plasmin, it helps stabilize blood clots and reduce bleeding.
Pharmacokinetics
Onset
Within 5-15 minutes after intravenous administration.
Excretion
Mainly excreted unchanged in the urine via glomerular filtration.
Half life
Approximately 2-3 hours.
Absorption
Rapidly absorbed after intravenous administration, reaching peak plasma concentrations within minutes.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Active intravascular clotting
- History of thrombosis or thromboembolic disease
- Subarachnoid hemorrhage (risk of cerebral edema and infarction)
- Severe renal impairment
- Hypersensitivity to tranexamic acid
Drug Interactions
Anticoagulants
Careful monitoring required.
Oral contraceptives
Increased risk of thrombosis.
Thrombolytic agents
Antagonistic effect.
Factor IX complex concentrates
Increased risk of thrombosis.
Storage
Store below 30°C. Do not freeze. Protect from light. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, orthostatic symptoms, hypotension, dizziness, headache, and convulsions. Management is supportive, including gastric lavage if ingested, and symptomatic treatment. Hemodialysis may be considered.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Tranexamic acid passes into breast milk, but amounts are generally considered too small to be harmful to a nursing infant. Consult a doctor.
