Overview of the medicine
Hepaximin 550 mg Tablet contains Rifaximin, a non-systemic antibiotic that works locally in the gastrointestinal tract. It is primarily used to treat hepatic encephalopathy, irritable bowel syndrome with diarrhea (IBS-D), and traveler's diarrhea.
Uses & Indications
Dosage
Adults
For Hepatic Encephalopathy: 550 mg orally two times a day. For IBS-D: 550 mg orally three times a day for 14 days, may be repeated twice.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment recommended in patients with renal impairment.
How to Take
May be taken with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Rifaximin is a rifamycin derivative antibiotic that inhibits bacterial RNA synthesis by binding to the beta-subunit of bacterial DNA-dependent RNA polymerase. Due to its poor systemic absorption, it acts locally in the gut to reduce ammonia-producing bacteria in hepatic encephalopathy, and to modify gut flora in IBS-D.
Pharmacokinetics
Onset
Varies by indication; for hepatic encephalopathy, symptomatic improvement may be seen within days.
Excretion
Primarily excreted unchanged in feces (96.9% of dose). Urinary excretion is negligible.
Half life
Approximately 5-6 hours in healthy volunteers, longer in hepatic impairment.
Absorption
Poorly absorbed systemically (<0.4%). Peak plasma concentrations are very low.
Metabolism
Minimally metabolized; some metabolism by hepatic CYP3A4, but limited due to poor absorption.

- Diarrhea complicated by fever and/or bloody stool

Warfarin
Potential to alter gut flora and affect vitamin K production, which may influence INR. Monitor INR closely.
P-glycoprotein inhibitors (e.g., cyclosporine)
May significantly increase systemic exposure to rifaximin due to increased absorption.
