Description
Overview of the medicine
Heximax 5 mg tablet contains Cetirizine Hydrochloride, a second-generation H1 receptor antagonist used for the relief of symptoms associated with perennial and seasonal allergic rhinitis, and chronic idiopathic urticaria.
Uses & Indications
Dosage
Adults
Usually 5 mg to 10 mg once daily, depending on severity of symptoms. Most common adult dose is 5 mg or 10 mg once daily.
Elderly
Typically 5 mg once daily. Dosage adjustment may be required in elderly patients with impaired renal function.
Renal_impairment
Dosage should be reduced to 5 mg once daily for patients with moderate renal impairment (creatinine clearance 31-60 mL/min). 5 mg every other day for severe renal impairment (creatinine clearance 11-30 mL/min). Contraindicated in end-stage renal disease (creatinine clearance <10 mL/min).
How to Take
Oral administration. May be taken with or without food.
Mechanism of Action
Cetirizine is a potent and highly selective H1-receptor antagonist. It works by blocking histamine, a natural substance that the body produces during an allergic reaction, thereby reducing allergy symptoms like itching, sneezing, and runny nose.

Within 20-60 minutes
Excretion
Primarily excreted unchanged in the urine (about 60% of the dose within 24 hours). A small amount is excreted in feces.
Half life
Approximately 8-11 hours (adults)
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are achieved within 1 hour.
Metabolism
About 10% of the dose is metabolized via O-dealkylation to a pharmacologically inactive metabolite.
Side Effects
Contraindications
- Hypersensitivity to cetirizine, hydroxyzine, or any piperazine derivatives.
- Severe renal impairment (creatinine clearance <10 mL/min).
Drug Interactions
Alcohol
Concurrent use with alcohol or other CNS depressants may lead to additive central nervous system depression, potentially impairing mental alertness and causing drowsiness.
Ritonavir
Ritonavir may increase plasma levels of cetirizine. Exercise caution.
Theophylline
A small decrease in cetirizine clearance was observed with co-administration of theophylline, but dosage adjustment is usually not necessary.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, sedation, tachycardia, tremor, and urinary retention. Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered soon after ingestion.

Pregnancy Category B. Use only if clearly needed. Cetirizine is excreted in breast milk; therefore, caution should be exercised when administered to a nursing mother.
