Hexiphen 2 mg Tablet contains Trihexyphenidyl Hydrochloride, an anticholinergic agent used to treat Parkinson's disease and drug-induced extrapyramidal disorders. It helps to reduce tremors, rigidity, and other involuntary movements by balancing neurotransmitters in the brain.

Dosage
Adults
Parkinson's disease: Initially, 1 mg once daily, increased by 2 mg increments at 3-5 day intervals until optimal response (usually 6-10 mg daily in 3-4 divided doses, max 15 mg daily). Drug-induced extrapyramidal disorders: Initially, 1 mg, then 5-15 mg daily in divided doses, depending on response.
Elderly
Start with a lower dose (e.g., 0.5 mg once daily) and titrate slowly due to increased sensitivity to anticholinergic effects.
Renal_impairment
Use with caution. Dosage reduction may be necessary in patients with severe renal impairment. Close monitoring is advised.
How to Take
Hexiphen tablets should be taken orally, usually with or after meals to minimize gastrointestinal irritation. The tablet can be taken with water.
Mechanism of Action
Trihexyphenidyl acts by blocking muscarinic acetylcholine receptors in the central nervous system, thereby reducing cholinergic overactivity. This helps to restore the balance between cholinergic and dopaminergic activity in the basal ganglia, which is disrupted in Parkinson's disease.

Side Effects
Contraindications
- •Angle-closure glaucoma.
- •Myasthenia gravis.

- •Tardive dyskinesia (relative contraindication, may exacerbate).
- •Hypersensitivity to Trihexyphenidyl or any component of the formulation.
Drug Interactions
Levodopa
May reduce the efficacy of levodopa by delaying gastric emptying.
Phenothiazines and other antipsychotics
May increase the risk of anticholinergic side effects or exacerbate tardive dyskinesia.
Anticholinergics (e.g., atropine, antihistamines, tricyclic antidepressants)
Increased anticholinergic effects, potentially leading to severe dry mouth, constipation, or urinary retention.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include CNS stimulation followed by depression, mydriasis, hot dry skin, flushing, hyperthermia, tachycardia, hypertension, and delirium. Treatment is supportive, including gastric lavage, activated charcoal, and symptomatic management. Physostigmine may be used as an antidote for severe central anticholinergic effects.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is not known whether trihexyphenidyl is excreted in human milk. Exercise caution when administering to a nursing mother.
