Description
Overview of the medicine
Hexirest 5 mg tablet contains Desloratadine, a non-drowsy antihistamine used to relieve allergy symptoms such as sneezing, runny nose, itchy or watery eyes, and hives. It provides fast and long-lasting relief from both seasonal and perennial allergic rhinitis and chronic idiopathic urticaria.
Uses & Indications
Dosage
Adults
5 mg (one tablet) once daily, with or without food.
Elderly
No dosage adjustment is generally required, but caution is advised due to potential age-related decrease in renal function.
Renal_impairment
For patients with severe renal impairment (creatinine clearance <20 mL/min), a starting dose of 5 mg every other day is recommended.
How to Take
Take Hexirest 5 mg tablet orally, with or without food. Swallow the tablet whole with a glass of water. Do not crush or chew it.
Mechanism of Action
Desloratadine is a long-acting, non-sedating, selective peripheral H1-receptor antagonist. It inhibits histamine release from mast cells and basophils. Additionally, it inhibits various inflammatory mediators from human mast cells, thereby reducing allergic inflammation.

Onset
Symptomatic relief usually begins within 30 minutes of administration.
Excretion
Excreted almost equally in urine and feces as conjugated metabolites.
Half life
Approximately 27 hours, allowing for once-daily dosing.
Absorption
Rapidly absorbed from the gastrointestinal tract; peak plasma concentrations reached approximately 3 hours after administration.
Metabolism
Extensively metabolized to 3-hydroxydesloratadine, an active metabolite, primarily by CYP3A4, although desloratadine is not a strong inhibitor or inducer of CYP enzymes.

Contraindications
- •Hypersensitivity to Desloratadine, Loratadine, or any component of the formulation.

Drug Interactions
Cimetidine
Increases plasma concentration of desloratadine.
Fluoxetine
Increases plasma concentration of desloratadine.
Erythromycin
Increases plasma concentration of desloratadine.
Ketoconazole
Increases plasma concentration of desloratadine.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In the event of an overdose, symptomatic and supportive treatment should be initiated. There is no specific antidote. Desloratadine is not removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Desloratadine is excreted in breast milk; therefore, a decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.
