Overview of the medicine
Hi-Trol 10 mg Tablet contains Carvedilol, which is a non-selective beta-adrenergic blocker with alpha-1 blocking activity. It is used to treat high blood pressure, angina, and heart failure.
Uses & Indications
Dosage
Adults
Hypertension: Initial 6.25 mg twice daily, increasing to 12.5 mg then 25 mg twice daily. Angina: Initial 12.5 mg twice daily, increasing to 25 mg twice daily. Heart Failure: Initial 3.125 mg twice daily, increasing gradually to a maximum of 25 mg twice daily (for patients <85 kg) or 50 mg twice daily (for patients >85 kg).
Elderly
No specific dose adjustment is required, but caution should be exercised.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Take orally, preferably with food to reduce the risk of orthostatic hypotension.
Mechanism of Action
Carvedilol blocks beta-1, beta-2, and alpha-1 adrenergic receptors. Beta-blockade reduces heart rate and contractility, lowering cardiac output. Alpha-1 blockade causes vasodilation, reducing peripheral vascular resistance. This combined action effectively lowers blood pressure and reduces cardiac workload.

Side Effects
Contraindications
- •Bronchial asthma or related bronchospastic conditions
- •Second or third-degree AV block (unless a permanent pacemaker is in place)
- •Sick sinus syndrome (unless a permanent pacemaker is in place)
- •Severe bradycardia
- •Cardiogenic shock
- •Decompensated heart failure requiring intravenous inotropic therapy
- •Severe hepatic impairment
- •Known hypersensitivity to carvedilol or any component of the formulation
Drug Interactions
Digoxin
Increased digoxin levels and enhanced bradycardia.
Clonidine
Potentiation of blood pressure and heart rate lowering effects. If both are discontinued, clonidine should be withdrawn gradually several days before the gradual withdrawal of carvedilol.
Rifampicin
Decreased plasma concentrations of carvedilol.
Insulin or oral hypoglycaemics
Potentiation of hypoglycaemic effect and masking of hypoglycaemia symptoms.
CYP2D6 inhibitors (e.g., Fluoxetine, Paroxetine, Quinidine)
Increased plasma concentrations of carvedilol.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe hypotension, bradycardia, heart failure, cardiogenic shock, and cardiac arrest. Treatment is symptomatic and supportive, including atropine for bradycardia, glucagon, and IV fluids/vasopressors for hypotension.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Carvedilol is excreted in breast milk, so breastfeeding is not recommended.
