Description
Overview of the medicine
Hi-Trol 5 mg syrup contains Bromhexine Hydrochloride, a mucolytic agent used to thin and loosen mucus in the airways, making it easier to cough up. It is used in the treatment of various respiratory conditions associated with excessive or thick mucus.
Uses & Indications
Dosage
Adults
Adults and adolescents over 12 years: 5-10 ml (5-10 mg) three times daily.
Elderly
No specific dosage adjustment required; use adult dose with caution.
Renal_impairment
In severe renal impairment, dose reduction may be necessary. Consult a doctor.
How to Take
Administer orally. Can be taken with or without food. Use the measuring cup or spoon provided for accurate dosing.
Mechanism of Action
Bromhexine works by stimulating serous glands, increasing the secretion of less viscous mucus. It also depolarizes lysosomal enzymes in goblet cells and activates acid hydrolyzing enzymes, leading to depolymerization of mucopolysaccharide fibers in the sputum. This action reduces the viscosity of sputum, aiding its clearance.
Pharmacokinetics
Onset
Approximately 30 minutes to 1 hour.
Excretion
Mainly excreted via the kidneys (approx. 85-90%) as metabolites, with only a small portion excreted unchanged.
Half life
Terminal elimination half-life: approximately 15-20 hours (due to extensive tissue distribution and slow release from tissues). The effective half-life for mucolytic action is shorter, around 6-8 hours.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract. Undergoes extensive first-pass metabolism.
Metabolism
Extensively metabolized in the liver to various hydroxylated metabolites, including ambroxol, which is also pharmacologically active.
Side Effects
Contraindications
- •Hypersensitivity to bromhexine or any component of the formulation.
- •Peptic ulcer (due to potential for increased gastric secretion, though risk is generally low at therapeutic doses).
Drug Interactions
Antibiotics
Bromhexine may enhance the penetration of certain antibiotics (e.g., amoxicillin, erythromycin, oxytetracycline) into bronchial secretions and lung tissue, potentially improving their efficacy.
Antitussives
Concomitant use with antitussives (cough suppressants) is generally not recommended as it may lead to a dangerous build-up of secretions due to inhibited cough reflex.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and may include gastrointestinal disturbances like nausea, vomiting, diarrhea. Management is supportive and symptomatic; gastric lavage or emesis may be considered if recent ingestion. No specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy and lactation only if clearly needed and potential benefits outweigh potential risks. Consult a doctor. Limited data available.
