Overview of the medicine
Hipre 10 mg Tablet contains Escitalopram, which is an antidepressant belonging to the Selective Serotonin Reuptake Inhibitor (SSRI) class. It is primarily used to treat various mental health conditions, including major depressive disorder and generalized anxiety disorder.
Uses & Indications
Dosage
Adults
Initial dose: 10 mg once daily. May be increased to 20 mg once daily after at least one week, based on clinical response. Max dose: 20 mg/day.
Elderly
Initial dose: 5 mg once daily. May be increased to 10 mg once daily after at least one week. Max dose: 10 mg/day.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Escitalopram selectively inhibits the reuptake of serotonin (5-HT) in the central nervous system, leading to an potentiation of serotonergic activity in the brain. This results in an antidepressant effect and helps in managing anxiety disorders.
Pharmacokinetics
Onset
Initial antidepressant effects may be observed within 2-4 weeks; full effects may take longer.
Excretion
Mainly through hepatic metabolism, followed by renal and fecal excretion. Approximately 8% of the dose is excreted unchanged in urine.
Half life
Approximately 27-32 hours (single dose), extending to 50 hours with multiple doses.
Absorption
Well absorbed orally, bioavailability is about 80%. Absorption is not affected by food.
Metabolism
Extensively metabolized in the liver by CYP2C19, CYP3A4, and CYP2D6 to its primary metabolites, S-desmethylcitalopram and S-didesmethylcitalopram.
Side Effects
Contraindications
- Hypersensitivity to escitalopram or any component of the formulation.
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI therapy due to risk of serotonin syndrome.
- Concomitant use with pimozide (due to QT prolongation risk).
Drug Interactions
MAOIs
Risk of serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability).
Alcohol
Enhances CNS depressive effects; avoid concomitant use.
Pimozide
Increased pimozide levels and QT prolongation.
Triptans
Increased risk of serotonin syndrome.
Warfarin
Increased risk of bleeding due to potential changes in INR.
NSAIDs/Aspirin
Increased risk of gastrointestinal bleeding.

Symptoms of overdose include dizziness, tremor, agitation, convulsion, somnolence, coma, nausea, vomiting, tachycardia, hypotension, and ECG changes (e.g., QT prolongation). Treatment is supportive and symptomatic; maintain airway, monitor cardiac and vital signs. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Escitalopram is excreted in human milk; advise caution when administered to a nursing woman.
