Overview of the medicine
Hipre 5 mg Tablet contains Amlodipine, a calcium channel blocker used to treat high blood pressure (hypertension) and angina (chest pain). It works by relaxing blood vessels, which lowers blood pressure and increases the supply of blood and oxygen to the heart.
Uses & Indications
Dosage
Adults
For Hypertension and Angina: Initially 5 mg once daily. Max 10 mg once daily.
Elderly
Start with a lower dose of 2.5 mg once daily, titrate upwards carefully.
Renal_impairment
No dosage adjustment is typically required for patients with renal impairment.
How to Take
Take orally with or without food, usually once daily.
Mechanism of Action
Amlodipine blocks L-type calcium channels in the smooth muscle cells of blood vessels and the heart. This leads to vasodilation (widening of blood vessels), decreased peripheral vascular resistance, and reduced cardiac afterload, resulting in lower blood pressure. In angina, it increases myocardial oxygen supply by dilating coronary arteries.

Onset
Antihypertensive effect begins within 6-12 hours.
Excretion
Excreted primarily via urine (60% as inactive metabolites, 10% as unchanged drug) and 20-25% via feces.
Half life
Plasma elimination half-life is 30-50 hours, allowing for once-daily dosing.
Absorption
Well absorbed after oral administration, with peak plasma concentrations reached 6-12 hours post-dose. Bioavailability is approximately 64-90%. Food does not affect absorption.
Metabolism
Extensively metabolized in the liver to inactive metabolites (approximately 90%) via CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Amlodipine or other dihydropyridine derivatives
- Severe hypotension
- Cardiogenic shock
- Obstruction of the outflow tract of the left ventricle (e.g., high grade aortic stenosis)
- Hemodynamically unstable heart failure after acute myocardial infarction
Drug Interactions
Simvastatin
Amlodipine increases simvastatin exposure. Limit simvastatin dose to 20 mg daily when co-administered with amlodipine.
CYP3A4 Inducers
Co-administration with CYP3A4 inducers (e.g., rifampicin, St. John's Wort) may decrease amlodipine plasma concentration.
CYP3A4 Inhibitors
Co-administration with strong or moderate CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, clarithromycin, ritonavir, diltiazem) may increase amlodipine exposure, requiring dose reduction.
Ciclosporin/Tacrolimus
Increased systemic exposure to ciclosporin/tacrolimus when co-administered with amlodipine; monitor levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms: Profound and prolonged systemic hypotension with possible reflex tachycardia. Management: Symptomatic and supportive. If recent ingestion, gastric lavage. Administer activated charcoal. Clinically significant hypotension requires active cardiovascular support including monitoring of cardiac and respiratory function, elevation of extremities, and administration of fluids. Vasopressors (e.g., norepinephrine) may be helpful. Intravenous calcium gluconate may reverse the effects of calcium channel blockade.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Small amounts of amlodipine are excreted in human milk; caution should be exercised when amlodipine is administered to a nursing woman.
