Description
Overview of the medicine
Hitflam 1% w/w Gel is a non-steroidal anti-inflammatory drug (NSAID) used for the topical treatment of pain and inflammation associated with various musculoskeletal conditions.
Uses & Indications
Dosage
Adults
Apply a small amount (2-4g, size of a cherry or walnut depending on the affected area) to the affected area 3-4 times daily and rub gently into the skin.
Elderly
No specific dose adjustment required; use as per adult dosage.
Renal_impairment
No specific dose adjustment required as systemic absorption is minimal.
How to Take
For external use only. Apply thinly to the affected skin area and rub in gently. Do not apply to open wounds, infected skin, or mucous membranes.
Mechanism of Action
Inhibits cyclooxygenase (COX-1 and COX-2) enzymes, leading to a reduction in prostaglandin synthesis, thereby reducing pain and inflammation.
Pharmacokinetics
Onset
Pain relief typically within 30-60 minutes.
Excretion
Mainly via urine (metabolites) and bile.
Half life
Systemic plasma half-life is about 1-2 hours; local therapeutic effects persist longer.
Absorption
Limited systemic absorption; high local tissue concentration achieved.
Metabolism
Primarily hepatic, mainly by CYP2C9.
Side Effects
Contraindications
- Hypersensitivity to diclofenac, aspirin, or other NSAIDs.
- Patients with a history of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
- Application to broken skin, open wounds, infected skin, or eczema.
Drug Interactions
Methotrexate
Possible increase in methotrexate levels, though clinically significant interaction is unlikely with topical use.
Oral anticoagulants (e.g., warfarin)
Increased risk of bleeding, though systemic absorption is minimal.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Overdose is unlikely with topical application due to minimal systemic absorption. If excessive amounts are applied, wipe off the excess and wash the area. In case of accidental ingestion, seek medical attention immediately.
Pregnancy & Lactation
Avoid in the third trimester of pregnancy. Use with caution in the first and second trimesters and during lactation, only if the potential benefit justifies the potential risk to the fetus/infant. Consult a doctor.
