Description
Overview of the medicine
Hitflam 50 mg Tablet is a non-steroidal anti-inflammatory drug (NSAID) used for pain relief, inflammation, and fever. It is commonly prescribed for conditions like arthritis, muscle pain, dental pain, and menstrual cramps.
Uses & Indications
Dosage
Adults
Usually 50 mg 2-3 times daily, or as directed by physician. Maximum 150 mg/day.
Elderly
Use with caution; lower doses may be necessary due to increased risk of side effects.
Renal_impairment
Use with caution in mild to moderate renal impairment; avoid in severe renal impairment.
How to Take
Take orally, preferably after meals with a full glass of water, to minimize gastrointestinal upset. Do not crush or chew the tablet.
Mechanism of Action
Diclofenac inhibits the cyclooxygenase (COX-1 and COX-2) enzymes, which are responsible for the synthesis of prostaglandins. Prostaglandins mediate inflammation, pain, and fever.
Pharmacokinetics
Onset
Typically within 30-60 minutes for pain relief.
Excretion
Approximately 60% excreted in urine as metabolites, and 30-40% excreted in bile/faeces.
Half life
Approximately 1-2 hours (terminal half-life of parent drug), but its pharmacological effects last longer due to accumulation in synovial fluid.
Absorption
Rapid and complete absorption after oral administration, though food can delay it. Peak plasma concentrations are reached in 2-3 hours.
Metabolism
Extensively metabolized in the liver via glucuronidation and hydroxylation, primarily by CYP2C9.
Side Effects
Contraindications
- Known hypersensitivity to diclofenac or other NSAIDs
- Active gastrointestinal ulceration, bleeding, or perforation
- Severe heart failure
- Severe renal or hepatic impairment
- Third trimester of pregnancy
- Coronary artery bypass graft (CABG) surgery
Drug Interactions
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Diuretics and ACE inhibitors
Reduced hypotensive effect and increased risk of renal impairment.
Lithium, Methotrexate, Digoxin
Increased plasma levels of these drugs, leading to toxicity.
Warfarin and other anticoagulants
Increased risk of bleeding.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Management involves supportive and symptomatic treatment. Gastric lavage and activated charcoal may be considered within an hour of ingestion.
Pregnancy & Lactation
Avoid during the third trimester of pregnancy due to risk of premature closure of the fetal ductus arteriosus. Use with caution during first and second trimesters and during lactation, only if clearly needed and benefits outweigh risks. Consult a doctor.
