Description
Overview of the medicine
Hysomide 20 mg Injection is used for the symptomatic treatment of nausea and vomiting of various origins, including post-operative nausea and vomiting, chemotherapy-induced nausea and vomiting, and for improving gastric emptying in gastroparesis.
Uses & Indications
Dosage
Adults
20 mg (1 ampoule) IM or IV, 6 to 8 hourly as required. Max 60 mg/day.
Elderly
A reduced dose of 10 mg IM or IV, 6 to 8 hourly may be considered, particularly in patients with impaired renal or hepatic function.
Renal_impairment
Creatinine clearance < 10 mL/min: 50% of the normal dose. Creatinine clearance 10-50 mL/min: 75% of the normal dose.
How to Take
Hysomide Injection can be administered intramuscularly (IM) or slowly intravenously (IV). For IV administration, inject slowly over at least 2 minutes to minimize the risk of acute dystonic reactions.
Mechanism of Action
Hysomide acts primarily as a dopamine D2 receptor antagonist in the chemoreceptor trigger zone (CTZ) in the brain, thereby preventing nausea and vomiting. It also enhances gastrointestinal motility and tone by sensitizing muscarinic receptors and increasing acetylcholine release from cholinergic neurons, leading to increased gastric emptying and intestinal transit.
Pharmacokinetics
Onset
Within 1-3 minutes after intravenous (IV) administration; 10-15 minutes after intramuscular (IM) administration.
Excretion
Mainly excreted in urine, with a small portion excreted in feces.
Half life
Approximately 4-6 hours.
Absorption
Rapid and complete absorption after intramuscular (IM) administration. Bioavailability is high.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation pathways.
Side Effects
Contraindications
- •Hypersensitivity to Hysomide or any component of the formulation.
- •Gastrointestinal hemorrhage, obstruction, or perforation.
- •Pheochromocytoma (due to risk of hypertensive crisis).
- •Epilepsy, as it may increase seizure frequency.
- •History of tardive dyskinesia from dopamine antagonist treatment.
Drug Interactions
Dopaminergic agents (e.g., Levodopa)
Hysomide may antagonize the effects of dopaminergic agents.
SSRIs (e.g., Fluoxetine, Sertraline)
Increased risk of serotonin syndrome, especially with high doses or prolonged use.
Anticholinergics and Opioid Analgesics
May antagonize the effects of Hysomide on GI motility.
CNS Depressants (e.g., alcohol, sedatives, hypnotics)
May enhance the sedative effects of Hysomide.
Storage
Store in a cool, dry place below 30°C. Protect from light. Do not freeze.
Overdose
Symptoms of overdose may include drowsiness, disorientation, and extrapyramidal reactions (e.g., muscle rigidity, involuntary movements). Management involves symptomatic and supportive care. Anticholinergic drugs (e.g., diphenhydramine) or benzodiazepines may be used to control extrapyramidal symptoms.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Hysomide is excreted into breast milk; caution should be exercised when administered to a nursing mother.
