Description
Overview of the medicine
Indapril 2 mg Tablet is a thiazide-like diuretic used primarily to treat high blood pressure (hypertension) and edema (fluid retention) associated with heart failure, liver cirrhosis, and kidney disorders. It works by increasing the excretion of sodium and water from the body, thereby reducing blood volume and blood pressure.
Uses & Indications
Dosage
Adults
For hypertension: Initially 2 mg once daily, preferably in the morning. May be increased to 2.5 mg or 5 mg if needed, after consulting a doctor. For edema: Initially 2.5 mg daily, adjust based on response.
Elderly
No specific dose adjustment is generally required, but close monitoring of renal function and electrolytes is advised.
Renal_impairment
Not recommended in severe renal impairment (creatinine clearance <30 mL/min). In moderate impairment, careful monitoring is necessary. Generally, a lower starting dose might be considered.
How to Take
Take the tablet orally once daily in the morning, preferably with food, to minimize gastrointestinal discomfort. Do not crush or chew the tablet; swallow it whole with a glass of water.
Mechanism of Action
Indapamide acts on the distal convoluted tubule of the kidney, inhibiting the reabsorption of sodium and chloride. This leads to increased excretion of sodium, chloride, and water, along with some potassium and magnesium. It also has a direct vasodilatory effect, reducing peripheral vascular resistance.
Pharmacokinetics
Onset
Within 2 hours (diuretic effect), full antihypertensive effect may take several days.
Excretion
Mainly via urine (70% as metabolites) and feces (22% as metabolites).
Half life
Approximately 14-18 hours.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract (bioavailability approx. 93%). Food may slightly delay absorption but does not affect the total amount absorbed.
Metabolism
Extensively metabolized in the liver, primarily by hydroxylation and glucuronidation. Less than 5% excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to indapamide or sulfonamide derivatives
- •Severe renal impairment (anuria)
- •Severe hepatic encephalopathy or severe hepatic impairment
- •Hypokalemia
Drug Interactions
Lithium
Increased lithium plasma levels and toxicity.
Cardiac glycosides
Hypokalemia may potentiate the toxic effects of cardiac glycosides.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
Reduced antihypertensive and diuretic effects of indapamide, risk of acute renal failure.
ACE inhibitors/Angiotensin II receptor antagonists
Increased risk of sudden hypotension and/or acute renal failure at the start of treatment.
Corticosteroids, Laxatives, Amphotericin B (systemic)
Increased risk of hypokalemia.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, weakness, electrolyte disturbances (hypokalemia, hyponatremia), and marked hypotension. Management is symptomatic and supportive, including gastric lavage, correction of fluid and electrolyte imbalances.
Pregnancy & Lactation
Not recommended during pregnancy due to potential for fetal ischemia and hypovolemia. Avoid during lactation as it is excreted in breast milk and may suppress lactation and cause adverse effects in the infant.
