Overview of the medicine
Indapril 4 mg tablet is an angiotensin-converting enzyme (ACE) inhibitor used primarily to treat high blood pressure (hypertension) and heart failure. It works by relaxing blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
Initial dose 4 mg orally once daily. May be increased up to 16 mg daily depending on patient response.
Elderly
Initial dose 2 mg orally once daily, adjusting based on blood pressure response and renal function.
Renal_impairment
Starting dose 2 mg orally once daily for CrCl < 30 mL/min, then titrate carefully.
How to Take
Take orally once daily, with or without food. It is recommended to take it at the same time each day.
Mechanism of Action
Indapril inhibits the angiotensin-converting enzyme (ACE), preventing the conversion of angiotensin I to angiotensin II, a potent vasoconstrictor. This leads to reduced vasoconstriction, decreased aldosterone secretion, and ultimately lowered blood pressure and reduced cardiac preload/afterload.

Absorption

Primarily metabolized in the liver to its active metabolite, indaprilat, through hydrolysis.
Side Effects
Contraindications
- •History of angioedema related to previous ACE inhibitor treatment
- •Hereditary or idiopathic angioedema
- •Concomitant use with aliskiren in patients with diabetes or renal impairment
- •Pregnancy (especially second and third trimesters)
Drug Interactions
NSAIDs
May reduce antihypertensive effect and increase risk of renal impairment.
Lithium
Increased serum lithium levels and risk of toxicity. Monitor lithium levels.
Aliskiren
Contraindicated in diabetic or renally impaired patients due to increased risk of renal impairment, hyperkalemia, and hypotension.
Diuretics
Increased hypotensive effect. Monitor blood pressure and renal function.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia. Monitor serum potassium.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include severe hypotension, shock, bradycardia, electrolyte disturbances, and renal failure. Treatment involves symptomatic and supportive measures, including intravenous fluid infusion, vasopressors, and hemodialysis if severe.
Pregnancy & Lactation
Pregnancy Category D (first trimester) to X (second and third trimesters). Use is contraindicated during the second and third trimesters. Not recommended during breastfeeding as it may pass into breast milk.
