Overview of the medicine
Inoxon 500 mg Injection is a broad-spectrum carbapenem antibiotic used to treat a wide range of severe bacterial infections in adults and children, including pneumonia, urinary tract infections, intra-abdominal infections, skin and soft tissue infections, meningitis, and sepsis. It contains Meropenem and is administered intravenously.
Uses & Indications
Dosage
Adults
Typically 500 mg to 1 g intravenously every 8 hours, depending on the severity and type of infection. For bacterial meningitis or severe infections, 1 g every 8 hours is common.
Elderly
Dosage adjustment may be necessary based on renal function.
Renal_impairment
Reduced dosage is required. For CrCl 26-50 mL/min: 500 mg every 12 hours; for CrCl 10-25 mL/min: 250 mg every 12 hours; for CrCl <10 mL/min: 250 mg every 24 hours.
How to Take
Administered by slow intravenous (IV) injection over 3-5 minutes or by intravenous infusion over 15-30 minutes. Reconstitution instructions must be followed carefully by a healthcare professional.
Mechanism of Action
Meropenem exerts its bactericidal activity by interfering with bacterial cell wall synthesis. It readily penetrates the bacterial cell wall to reach penicillin-binding protein (PBP) targets, causing inhibition of cell wall synthesis and subsequent bacterial cell lysis and death.

Primarily excreted unchanged in the urine (approximately 70% within 12 hours).

Rapid and complete absorption after intravenous administration.
Metabolism
Minimal hepatic metabolism, primarily hydrolyzed by dehydropeptidase-I (DHP-I), though meropenem is relatively stable to DHP-I.
Side Effects
Contraindications
- Hypersensitivity to meropenem, any other carbapenem antibiotic, or any component of the formulation
- Severe hypersensitivity (e.g., anaphylactic reaction, severe skin reaction) to any other beta-lactam antibacterial agent (e.g., penicillins or cephalosporins)
Drug Interactions
Probenecid
Inhibits renal tubular secretion of meropenem, increasing its plasma concentration and prolonging its half-life. Concomitant administration is not recommended.
Valproic Acid/Sodium Valproate
May significantly decrease valproic acid plasma concentrations, leading to loss of seizure control. Concomitant use is generally not recommended.
Storage
Store below 25°C in the original package to protect from light. Do not freeze. Reconstituted solutions should be used immediately or stored according to specific guidelines (e.g., for a limited time under refrigeration).
Overdose
Limited experience with overdose. Symptoms may include nausea, vomiting, diarrhea, seizures. Treatment is symptomatic and supportive. Hemodialysis can remove meropenem from the circulation.
Pregnancy & Lactation
Pregnancy category B. Use during pregnancy only if clearly needed. Meropenem is excreted in small amounts in human milk; use with caution during lactation.
