Ipadox is an antiparasitic combination medication containing Ivermectin and Albendazole, used to treat various helminth infections.
Uses & Indications
Dosage
Adults
Typically a single oral dose of Ivermectin 6 mg + Albendazole 400 mg. Dosage may vary based on specific infection and body weight; follow physician's instructions.
Elderly
Generally same as adults; use with caution due to potential age-related organ function decline.
Renal_impairment
No specific dose adjustment recommended, but use with caution.
How to Take
Take orally with water, preferably with a meal to enhance absorption of Albendazole.
Mechanism of Action
Ivermectin binds to glutamate-gated chloride channels, causing paralysis and death of parasites. Albendazole inhibits microtubule formation, leading to impaired glucose uptake and death of helminths.

Onset
Clinical effects may be observed within days, depending on the infection.
Excretion
Ivermectin is primarily excreted in feces. Albendazole sulfoxide and its metabolites are mainly excreted in urine.
Half life
Ivermectin: 12-36 hours. Albendazole sulfoxide: 8-12 hours.
Absorption
Ivermectin is well absorbed. Albendazole is poorly absorbed but rapidly metabolized to its active sulfoxide metabolite, with absorption enhanced by fatty meals.
Metabolism
Ivermectin is metabolized in the liver (CYP3A4). Albendazole is extensively metabolized in the liver to albendazole sulfoxide, which is the primary active metabolite.
Side Effects
Contraindications

Drug Interactions
Warfarin
Ivermectin may increase the anticoagulant effect of warfarin.
CNS depressants
Ivermectin may enhance the effects of CNS depressants.
Cimetidine, Dexamethasone, Praziquantel
These drugs may increase plasma levels of albendazole sulfoxide.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. Gastric lavage may be considered soon after ingestion.
Pregnancy & Lactation
Albendazole is pregnancy category C/D and should be avoided in the first trimester. Use in lactation only if benefits outweigh risks. Consult a doctor.
