Description
Overview of the medicine
Ipralin (Ipratropium Bromide) is an anticholinergic bronchodilator used for the maintenance treatment of bronchospasm associated with chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema.
Uses & Indications
Dosage
Adults
Usually 2 puffs (40 mcg) four times a day, not exceeding 12 puffs in 24 hours.
Elderly
No specific dose adjustment is required, but caution is advised.
Renal_impairment
No dose adjustment needed, as systemic absorption is minimal.
How to Take
For oral inhalation only. Shake well before use. Prime the inhaler before first use or if not used for a long time.
Mechanism of Action
Ipratropium bromide is an anticholinergic (parasympatholytic) agent which, by local action, inhibits vagally mediated reflexes by antagonizing the action of acetylcholine at the muscarinic receptors on bronchial smooth muscle. This results in bronchodilation.

Absorption
Poorly absorbed systemically following oral inhalation. Less than 20% of the dose reaches the lower airways, and a small portion is absorbed into systemic circulation.
Metabolism
Hydrolyzed to inactive metabolites via esterases.

Contraindications

Storage
Store below 30°C. Protect from freezing and direct sunlight. Do not puncture or incinerate canister.
Overdose
Symptoms include dry mouth, blurred vision, and tachycardia. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed during pregnancy. Excretion into human milk is unknown, use with caution in nursing mothers.
