Description
Overview of the medicine
Ipsavent 25 mg Respirator Solution contains Ipratropium Bromide, an anticholinergic bronchodilator. It is used to relieve bronchospasm associated with chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema. While the strength of 25 mg is high, it may indicate a concentrated solution for dilution or specific nebulizer settings. Always consult the product leaflet for specific dosing instructions.
Uses & Indications
Dosage
Adults
The typical dose of Ipratropium Bromide for nebulization is 250-500 mcg (0.25-0.5 mg) three to four times daily. For a 25 mg/mL solution, this would require significant dilution or a very small volume. Always refer to the specific product insert and physician's instructions for the 25 mg strength.
Elderly
No specific dose adjustment is generally required, but caution is advised in elderly patients with prostatic hyperplasia or narrow-angle glaucoma due to anticholinergic effects.
Renal_impairment
No specific dose adjustment necessary for mild to moderate renal impairment. Data for severe impairment is limited.
How to Take
Ipsavent 25 mg Respirator Solution is intended for oral inhalation via a nebulizer. It must not be swallowed or injected. The solution may need to be diluted with normal saline before use, depending on the prescribed dose and the nebulizer device. Follow the instructions provided with your nebulizer and as directed by your physician.
Mechanism of Action
Ipratropium Bromide is an anticholinergic (parasympatholytic) agent that blocks muscarinic cholinergic receptors in the bronchial smooth muscle. This inhibition of acetylcholine's action prevents the increase in intracellular cyclic guanosine monophosphate (cGMP) levels caused by the interaction of acetylcholine with muscarinic receptors, leading to bronchodilation. It primarily works locally on the airways.
Pharmacokinetics
Onset
Bronchodilation begins within 15 minutes, with peak effects in 1-2 hours.
Excretion
Primarily excreted unchanged in feces (unabsorbed portion) and renally (absorbed portion).
Half life
Approximately 1.6 hours (terminal half-life of inhaled dose).
Absorption
Poorly absorbed from the gastrointestinal tract and lung surface. Most of the dose is deposited locally in the airways.
Metabolism
Partially metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Ipratropium Bromide, atropine or its derivatives (e.g., Atrovent, Combivent components).
- •Patients with allergies to soy lecithin or related food products (e.g., soybean, peanuts) if the formulation contains these.
Drug Interactions
Beta-agonists
Can be used concomitantly, often with synergistic bronchodilatory effects.
Other anticholinergics
Increased anticholinergic effects, caution advised.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose are typically mild and include dry mouth, blurred vision, and slight tachycardia. Management is generally supportive and symptomatic. Severe anticholinergic effects are unlikely due to poor systemic absorption.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but human data is limited. Use during pregnancy only if clearly needed. It is not known whether Ipratropium Bromide is excreted in human milk; use with caution in nursing mothers.
