Overview of the medicine
Isonil (Isosorbide Mononitrate) is a long-acting nitrate used to prevent attacks of angina pectoris (chest pain) in patients with coronary artery disease. It relaxes blood vessels, allowing more blood and oxygen to reach the heart.
Uses & Indications
Dosage
Adults
Initially 10 mg or 20 mg twice daily. May be increased to 20 mg or 40 mg twice daily. Doses should be taken 7 hours apart (e.g., 8 AM and 3 PM) to ensure a nitrate-free interval.
Elderly
No specific dose adjustment required, but start with lower dose and monitor carefully.
Renal_impairment
No dose adjustment usually necessary, but caution advised in severe renal impairment.
How to Take
Take orally with a glass of water, with or without food. To avoid tolerance, ensure a nitrate-free interval (typically 7-10 hours) between doses.
Mechanism of Action
Isosorbide Mononitrate is metabolized to nitric oxide (NO), which activates guanylyl cyclase, leading to increased cGMP. This results in smooth muscle relaxation, particularly in vascular smooth muscle, causing vasodilation of both arteries and veins. This reduces preload and afterload, decreasing myocardial oxygen demand.

Side Effects
Contraindications
- •Hypersensitivity to isosorbide mononitrate or other nitrates
- •Severe hypotension, cardiogenic shock, or uncompensated heart failure
- •Concomitant use with phosphodiesterase-5 inhibitors (e.g., sildenafil, tadalafil) due to risk of severe hypotension
Drug Interactions
Alcohol
May enhance hypotensive effect and cause dizziness or faintness.
Antihypertensive agents, diuretics
May potentiate hypotensive effects.
Phosphodiesterase-5 inhibitors (e.g., sildenafil, tadalafil)
Profound hypotension, potentially fatal. Concomitant use is absolutely contraindicated.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe hypotension, persistent throbbing headache, dizziness, palpitation, visual disturbances, flushing, nausea, vomiting, sweating, and syncope. Treatment is supportive, including elevation of legs, fluid administration, and vasopressors if needed.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs risk to the fetus. Excretion in breast milk is unknown; caution advised during breastfeeding.
