Description
Overview of the medicine
Isovent 200 mcg Tablet contains Salbutamol, a fast-acting bronchodilator used to relieve symptoms of asthma and chronic obstructive pulmonary disease (COPD). It helps open the airways in the lungs, making breathing easier.
Uses & Indications
Dosage
Adults
The usual starting dose for Salbutamol tablets is 2-4 mg (10-20 tablets of 200 mcg) 3-4 times daily. Dosage must be individualized by a physician.
Elderly
Lower initial doses may be required, e.g., 2 mg (10 tablets of 200 mcg) 3-4 times daily, adjusted according to response and tolerance.
Renal_impairment
No specific dose adjustment is generally required, but caution is advised in severe impairment due to potential accumulation of metabolites.
How to Take
Take Isovent 200 mcg Tablet orally, with or without food. Swallow the tablet whole with water. Do not crush, chew, or break the tablet.
Mechanism of Action
Salbutamol selectively stimulates beta-2 adrenergic receptors in bronchial smooth muscle, leading to relaxation of the muscle and bronchodilation. This helps to widen the airways and reduce airflow obstruction.
Pharmacokinetics
Onset
Within 30 minutes (oral)
Excretion
Primarily via urine, with about 60-70% excreted as unchanged drug and metabolites within 24 hours.
Half life
4-6 hours (oral)
Absorption
Rapidly and well-absorbed from the gastrointestinal tract.
Metabolism
Undergoes extensive first-pass metabolism in the liver to an inactive sulfate conjugate.
Side Effects
Contraindications
- Hypersensitivity to salbutamol or any excipients of the tablet.
- Threatened abortion during the first or second trimester of pregnancy (oral salbutamol is contraindicated as a tocolytic agent).
Drug Interactions
Diuretics
Concomitant use with potassium-depleting diuretics (e.g., loop or thiazide diuretics) may potentiate hypokalemia, increasing the risk of cardiac arrhythmias.
Beta-blockers
Non-selective beta-blockers (e.g., propranolol) may antagonize the bronchodilatory effect of salbutamol and can precipitate severe bronchospasm in asthmatic patients.
MAOIs and TCAs
Concomitant administration with monoamine oxidase inhibitors (MAOIs) or tricyclic antidepressants (TCAs) may potentiate the cardiovascular effects of salbutamol.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children. Do not freeze.
Overdose
Symptoms of overdose include tachycardia, tremors, headache, restlessness, hypokalemia, and metabolic acidosis. Management involves discontinuing the drug, providing supportive care, and monitoring electrolyte levels and cardiac function.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Salbutamol is excreted in breast milk; therefore, use with caution during lactation and only if recommended by a doctor.

YesClinical Trials
Numerous clinical trials have established the efficacy and safety of salbutamol for the symptomatic relief of bronchospasm in asthma and COPD. Studies confirm its rapid onset and short duration of action.
Lab Monitoring
- Monitor serum potassium levels, especially in patients receiving high doses, concomitant diuretics, or during acute severe asthma exacerbations.
Doctor Notes
- Emphasize the importance of patient education regarding the 'reliever' role of salbutamol and the need for regular 'controller' medication for persistent asthma.
- Monitor for signs of cardiovascular effects and hypokalemia, especially in susceptible patients.
- Advise patients not to increase dosage or frequency without medical consultation.
Patient Guidelines
- Do not exceed the prescribed dose or frequency without consulting your doctor.
- Seek medical attention immediately if your breathing problems worsen or if you need to use this medication more often than usual.
- Inform your doctor about all other medications, including over-the-counter drugs and herbal supplements.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Isovent 200 mcg Tablet may cause dizziness, tremors, or muscle cramps in some individuals. Exercise caution when driving or operating machinery, especially at the start of treatment or until you know how the medication affects you.
Lifestyle Advice
- Avoid smoking and exposure to allergens or irritants that may trigger asthma or COPD symptoms.
- Maintain a healthy diet and regular exercise routine as advised by your doctor to improve overall respiratory health.
