Description
Overview of the medicine
Ivacure 3 mg tablet is an anti-parasitic medication used to treat certain parasitic infections in humans, such as strongyloidiasis and onchocerciasis.
Uses & Indications
Dosage
Adults
Strongyloidiasis: A single oral dose of 200 mcg/kg body weight. Onchocerciasis: A single oral dose of 150 mcg/kg body weight every 6 to 12 months. Scabies: A single oral dose of 200 mcg/kg, repeated in 7-14 days if needed.
Elderly
No specific dose adjustments needed, but use with caution due to potential for comorbidities and polypharmacy. Dosing typically follows adult recommendations based on body weight.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Caution advised in severe renal impairment.
How to Take
Take the tablet orally with water on an empty stomach (at least 1 hour before or 2 hours after a meal).
Mechanism of Action
Ivermectin binds selectively and with high affinity to glutamate-gated chloride ion channels which occur in invertebrate nerve and muscle cells. This leads to an increase in the permeability of the cell membrane to chloride ions and subsequent hyperpolarization of the nerve or muscle cell, resulting in paralysis and death of the parasite.
Pharmacokinetics
Onset
Pharmacological effects may begin within hours, but clinical efficacy against parasites may take days to weeks depending on the infection.
Excretion
Excreted almost exclusively in the feces over approximately 12 days, with less than 1% excreted in the urine.
Half life
Plasma elimination half-life is approximately 12-36 hours, but can be longer.
Absorption
Readily absorbed after oral administration. Peak plasma concentrations are reached approximately 4-5 hours post-dose. Absorption can be enhanced with a high-fat meal.
Metabolism
Primarily metabolized in the liver, mainly by CYP3A4, into at least 10 metabolites.

- Hypersensitivity to Ivermectin or any component of the tablet.
- Not recommended for children under 15 kg body weight for most indications.
- Pregnancy and lactation (relative contraindication).
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, requiring close monitoring of INR.
CNS Depressants
Concurrent use with other CNS depressants may increase CNS depression (e.g., benzodiazepines, barbiturates).
CYP3A4 Inhibitors/Inducers
Drugs affecting CYP3A4 may alter ivermectin metabolism, though clinical significance is generally minor.

Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Ivermectin is excreted in human milk; caution should be exercised when administered to a nursing mother.
