Description
Overview of the medicine
Ivacure 6 mg Tablet contains Ivermectin, an antiparasitic medication used to treat various parasitic infections, including scabies, strongyloidiasis, onchocerciasis, and lymphatic filariasis. It works by paralyzing and killing parasites.
Uses & Indications
Dosage
Adults
Strongyloidiasis: 200 mcg/kg body weight as a single oral dose. Scabies: 200 mcg/kg body weight as a single oral dose, repeated after 7-14 days if needed. Onchocerciasis: 150 mcg/kg body weight as a single oral dose, usually repeated every 3-12 months.
Elderly
No specific dose adjustment is generally required, but caution is advised due to potential age-related decrease in hepatic function and polypharmacy.
Renal_impairment
No specific dose adjustment is recommended as less than 1% is excreted renally. However, use with caution in severe impairment.
How to Take
Take the tablet orally with water, usually on an empty stomach (e.g., 1 hour before a meal) for optimal absorption. For scabies, a single dose is often sufficient, but a second dose may be needed.
Mechanism of Action
Ivermectin selectively binds with high affinity to glutamate-gated chloride ion channels which occur in invertebrate nerve and muscle cells. This leads to an increase in the permeability of the cell membrane to chloride ions, resulting in hyperpolarization of the nerve or muscle cell, and subsequent paralysis and death of the parasite.
Pharmacokinetics
Onset
Within hours for antiparasitic effect, but clinical symptom resolution varies depending on the infection.
Excretion
Almost exclusively in feces over approximately 12 days, with less than 1% excreted in urine.
Half life
Approximately 12-36 hours (mean 18 hours).
Absorption
Well absorbed after oral administration; bioavailability is approximately 20-30% and can be increased with a high-fat meal.
Metabolism
Primarily hepatic, via CYP3A4, to various hydroxylated metabolites.
Side Effects
Contraindications
- Hypersensitivity to Ivermectin or any component of the tablet.
- Use in children weighing less than 15 kg.
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, requiring INR monitoring.
Other GABA agonists (e.g., benzodiazepines, barbiturates)
May potentiate GABAergic effects, leading to increased sedation or CNS depression.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include rash, edema, headache, dizziness, asthenia, nausea, vomiting, diarrhea, and abdominal pain. In severe cases, seizures and coma have been reported. Management involves supportive care and symptomatic treatment.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, use with caution during lactation or avoid breastfeeding for a period after dosing.
