Description
Overview of the medicine
Ivertin 12 mg tablet contains Ivermectin, an antiparasitic medication used to treat certain parasitic infections in humans, including onchocerciasis, strongyloidiasis, and scabies.
Uses & Indications
Dosage
Adults
Typical dose for strongyloidiasis is a single oral dose of 200 mcg/kg body weight. For onchocerciasis, 150 mcg/kg body weight as a single oral dose, repeated every 3 to 12 months. For scabies, a single oral dose of 200 mcg/kg body weight, potentially repeated after 7-14 days.
Elderly
Dosage considerations are generally similar to adults, but caution is advised due to potential decreased hepatic, renal, or cardiac function, and concomitant disease or other drug therapy.
Renal_impairment
No specific dosage adjustment recommended for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Take orally with water on an empty stomach (at least 1 hour before or 2 hours after a meal) for optimal absorption. Tablets can be crushed if necessary for easier administration.
Mechanism of Action
Ivermectin binds selectively and with high affinity to glutamate-gated chloride ion channels which occur in invertebrate nerve and muscle cells. This binding leads to an increase in the permeability of the cell membrane to chloride ions, causing hyperpolarization of the nerve or muscle cell, and results in paralysis and death of the parasite.
Pharmacokinetics
Onset
Effects typically observed within days for parasitic infections.
Excretion
Mainly excreted in feces (virtually exclusively), with less than 1% excreted in urine.
Half life
Approximately 12-36 hours.
Absorption
Well absorbed orally; peak plasma concentrations reached approximately 4-6 hours post-dose.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Ivermectin or any component of the tablet
- Pregnancy (unless potential benefits outweigh risks)
- Lactation (for some indications)
Drug Interactions
Warfarin
May enhance anticoagulant effect, increasing risk of bleeding. Monitor INR.
CNS depressants (e.g., benzodiazepines, barbiturates)
May potentiate CNS depression.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms of overdose may include rash, edema, headache, dizziness, nausea, vomiting, abdominal pain, diarrhea, seizures, and coma. Treatment is symptomatic and supportive, including fluid and electrolyte replacement, respiratory support, and blood pressure control.
Pregnancy & Lactation
Pregnancy: Category C. Use only if the potential benefit justifies the potential risk to the fetus. Lactation: Ivermectin is excreted in human milk. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
