Description
Overview of the medicine
Ivermectin is an antiparasitic medication used to treat certain parasitic infections, including onchocerciasis (river blindness), strongyloidiasis, lymphatic filariasis, and scabies. It works by paralyzing and killing the parasites.
Uses & Indications
Dosage
Adults
Onchocerciasis: 0.15 mg/kg as a single oral dose, repeated every 3-12 months. Strongyloidiasis: 0.2 mg/kg as a single oral dose. Scabies: 0.2 mg/kg as a single oral dose, repeated in 7-14 days if needed. Other parasitic infections: Dosage varies, typically 0.2 mg/kg single dose or daily for a few days.
Elderly
No specific dose adjustment generally needed, but monitor for adverse effects.
Renal_impairment
No specific dose adjustment recommended, as it is primarily excreted in feces.
How to Take
Oral. Take with a glass of water, preferably on an empty stomach (e.g., 1 hour before a meal) for optimal absorption, though can be taken with food to reduce GI upset.
Mechanism of Action
Ivermectin binds selectively and with high affinity to glutamate-gated chloride ion channels which are present in invertebrate nerve and muscle cells. This binding leads to an increase in the permeability of the cell membrane to chloride ions, causing hyperpolarization of the nerve or muscle cell, and results in paralysis and death of the parasite.

Primarily excreted in feces (approximately 99%), with less than 1% excreted in urine.

Absorption

Extensively metabolized in the liver, primarily by CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to ivermectin or any component of the tablet.
- Patients with impaired blood-brain barrier (e.g., meningitis, head trauma).
- Children weighing less than 15 kg.
Drug Interactions
Warfarin
May increase INR, monitor closely.
CYP3A4 inhibitors (e.g., ketoconazole, ritonavir)
May increase ivermectin plasma levels.
Other GABAergic drugs (e.g., benzodiazepines, barbiturates)
May enhance CNS depressant effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include rash, edema, headache, dizziness, asthenia, nausea, vomiting, diarrhea, abdominal pain, seizures, coma. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Excreted in breast milk, so advise caution or temporary discontinuation of breastfeeding.
