Description
Overview of the medicine
Ivermectin is an antiparasitic medication used to treat certain parasitic infections in humans, including onchocerciasis, strongyloidiasis, and scabies. It belongs to the macrocyclic lactone class of drugs.
Uses & Indications
Dosage
Adults
For Strongyloidiasis: A single oral dose of 200 mcg/kg body weight. For Onchocerciasis: A single oral dose of 150 mcg/kg body weight, typically repeated every 3-12 months. For Scabies: A single oral dose of 200 mcg/kg, repeated in 7-14 days if needed.
Elderly
No specific dose adjustment is generally required, but close monitoring for adverse effects is recommended due to potential age-related decline in hepatic and renal function.
Renal_impairment
No specific dosage adjustment is required for patients with renal impairment, but caution is advised. Ivermectin is primarily excreted in feces.
How to Take
Ivtin 6 mg Tablet should be taken orally with water on an empty stomach (at least 1 hour before or 2 hours after a meal) to maximize absorption and effectiveness.
Mechanism of Action
Ivermectin binds selectively and with high affinity to glutamate-gated chloride ion channels which occur in invertebrate nerve and muscle cells. This binding leads to an increase in the permeability of the cell membrane to chloride ions, resulting in hyperpolarization of the nerve or muscle cell, and subsequent paralysis and death of the parasite.
Pharmacokinetics
Onset
Clinical effects may be observed within hours to days depending on the type of parasitic infection.
Excretion
Excreted almost exclusively in the feces within 12 days, with less than 1% excreted in urine.
Half life
Approximately 12-56 hours.
Absorption
Well absorbed orally. Peak plasma concentrations are achieved approximately 4-5 hours after oral administration.
Metabolism
Primarily hepatic, mainly via cytochrome P450 3A4 (CYP3A4) isoenzymes.
Side Effects
Contraindications
- Known hypersensitivity to Ivermectin or any component of the tablet.
- Not recommended for children weighing less than 15 kg.
- Use with caution in pregnant women (except in mass drug administration programs where benefits outweigh risks).
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, requiring careful monitoring of INR.
Other CNS depressants (e.g., benzodiazepines, barbiturates)
May enhance CNS depressant effects, increasing the risk of dizziness and drowsiness.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include rash, edema, headache, dizziness, asthenia, nausea, vomiting, diarrhea, and abdominal pain. In severe cases, seizures, ataxia, and coma may occur. Treatment is symptomatic and supportive, including fluid and electrolyte replacement and maintaining blood pressure.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Ivermectin is excreted in human milk; caution should be exercised when administered to a nursing mother.

YesClinical Trials
Ivermectin has undergone extensive clinical trials for its approved indications. There have also been numerous trials exploring its efficacy in other parasitic diseases and, controversially, in COVID-19, with most large, high-quality trials showing no benefit for COVID-19.
Lab Monitoring
- Periodic stool examinations are recommended for patients being treated for strongyloidiasis to confirm eradication.
- In onchocerciasis, Mazzotti reaction (fever, pruritus, rash) monitoring is important, sometimes requiring corticosteroids.
- Liver function tests may be considered for prolonged or high-dose therapy, although liver toxicity is rare.
Doctor Notes
- Ensure proper diagnosis before prescribing, as Ivtin is specific to certain parasitic infections.
- Advise patients on the importance of taking the medication on an empty stomach.
- Educate patients about potential Mazzotti reactions in Onchocerciasis and how to manage them.
- Counsel against off-label use, especially for viral infections like COVID-19.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor.
- Do not stop taking the medicine early, even if you feel better.
- Report any unusual or severe side effects to your doctor immediately.
- Take on an empty stomach with water.
Missed Dose Advice
Since Ivtin is often taken as a single dose or infrequently, take the missed dose as soon as you remember. If it is almost time for your next scheduled dose, skip the missed dose and resume your regular dosing schedule. Do not double doses.
Driving Precautions
Ivtin may cause dizziness or drowsiness. Patients should be cautioned about operating machinery or driving if they experience these symptoms.
Lifestyle Advice
- Practice good personal hygiene to prevent reinfection, especially for parasitic skin conditions like scabies.
- Follow up with your doctor as advised to ensure the infection has been cleared and for any necessary re-treatment.
