Description
Overview of the medicine
Jefenac-1-w-gel is a topical non-steroidal anti-inflammatory drug (NSAID) containing Diclofenac Diethylamine equivalent to 1% w/w Diclofenac Sodium. It is used for the local symptomatic relief of pain and inflammation in conditions like osteoarthritis, acute strains, sprains, and contusions.
Uses & Indications
Dosage
Adults
Apply a small amount (e.g., 2-4 cm strip) to the affected area 3-4 times daily, gently rubbing it into the skin. The amount applied should be sufficient to cover the affected area. Do not exceed 8 grams per day over a single body area.
Elderly
No specific dose adjustment is generally required, but caution is advised due to potential for increased sensitivity.
Renal_impairment
Topical application results in minimal systemic absorption, thus dose adjustment is usually not necessary. However, use with caution in severe renal impairment.
How to Take
For external use only. Apply a thin layer of gel to the affected skin area and gently rub it in. Wash hands thoroughly after application, unless the hands are the treated area. Do not cover with occlusive bandages.
Mechanism of Action
Diclofenac works by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes, which are responsible for the synthesis of prostaglandins. Prostaglandins are mediators of inflammation, pain, and fever. By reducing prostaglandin synthesis, diclofenac alleviates pain and inflammation. When applied topically, it exerts its effects locally at the site of application.

Onset
Local pain relief within 30-60 minutes, peak effect may take a few days with consistent application.
Excretion
Mainly renal (approximately 60%) and biliary (approximately 35%) excretion of metabolites.
Half life
Systemic half-life is approximately 1-2 hours, but local tissue retention can be longer.
Absorption
Systemic absorption is low following topical application, typically less than 10% of that achieved with oral administration. Plasma concentrations are detectable but significantly lower than oral doses.
Metabolism
Primarily hepatic via CYP2C9 and CYP2C19 enzymes, forming hydroxylated metabolites.
Side Effects
Contraindications
- •Hypersensitivity to diclofenac, aspirin, other NSAIDs, or any components of the gel.
- •Patients with asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
- •Application to open wounds, infected skin, or dermatitis.
- •Third trimester of pregnancy.
Drug Interactions
Oral NSAIDs
Concomitant use with oral NSAIDs may increase the risk of systemic side effects, although minimal.
Lithium/Methotrexate
Increased plasma levels, though systemic absorption is low.
Diuretics/ACE inhibitors
Reduced efficacy of these drugs due to prostaglandin inhibition.
Anticoagulants (e.g., Warfarin)
Potentially increased risk of bleeding, though systemic absorption is low.

Store below 30°C. Do not freeze. Keep out of reach of children.
Overdose
Due to low systemic absorption, an overdose from topical application is unlikely. If accidentally ingested or if excessive amounts are applied over large areas, systemic adverse effects similar to oral NSAIDs might occur. Treatment is supportive and symptomatic; gastric emptying might be considered if recent ingestion.
Pregnancy & Lactation
Pregnancy: Category C in first and second trimesters, D in third trimester (due to risk of premature closure of fetal ductus arteriosus). Use during the first two trimesters only if clearly needed. Contraindicated in the third trimester. Lactation: Diclofenac is excreted in breast milk in small amounts after oral administration. Systemic absorption after topical application is low, so risk to breastfed infant is considered low, but use with caution and under medical advice.
