Description
Overview of the medicine
Gabapentin is an anti-epileptic and analgesic drug used to treat partial seizures and neuropathic pain, such as postherpetic neuralgia.
Uses & Indications
Dosage
Adults
For partial seizures: Initial dose 300 mg three times daily, may be titrated up to 1800-3600 mg/day. For postherpetic neuralgia: Initial dose 300 mg on day 1, 300 mg twice daily on day 2, 300 mg three times daily on day 3, then titrate as needed.
Elderly
Lower initial dose recommended, and dosage should be adjusted based on renal function.
Renal_impairment
Dose must be adjusted based on creatinine clearance (CrCl). For CrCl 30-59 mL/min, total daily dose should be 400-800 mg. For CrCl 15-29 mL/min, total daily dose should be 200-500 mg. For CrCl <15 mL/min, total daily dose should be 100-300 mg.
How to Take
Joytrip-300-mcg-tablet should be taken orally, with or without food. When taking for seizures, maintain a consistent dosing interval (e.g., three times daily with no more than 12 hours between doses).
Mechanism of Action
The exact mechanism of action is not fully understood, but Gabapentin binds with high affinity to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system.

Contraindications

Drug Interactions
Opioids
Increased risk of respiratory depression, sedation, and coma. Use with caution, and reduce gabapentin dose if needed.
Antacids (containing aluminum and magnesium)
May decrease gabapentin bioavailability by up to 24%. Administer gabapentin at least 2 hours after antacid administration.
CNS Depressants (e.g., alcohol, benzodiazepines)
May potentiate CNS side effects like drowsiness and dizziness.
Storage
Store at room temperature (15-30°C or 59-86°F). Protect from moisture and light. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, double vision, slurred speech, drowsiness, lethargy, and mild diarrhea. Management is primarily supportive; hemodialysis can be used in severe cases, especially in patients with renal impairment.
Pregnancy & Lactation
Pregnancy Category C. Gabapentin has been shown to be teratogenic in animals. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Gabapentin is excreted in human milk; caution should be exercised when gabapentin is administered to a nursing woman.
