Description
Overview of the medicine
K-Nor is a potassium supplement used to treat or prevent low blood potassium (hypokalemia), a condition that can occur due to various reasons like certain illnesses, medications, or prolonged vomiting or diarrhea. Potassium is a vital electrolyte necessary for heart function, muscle contraction, and nerve signaling.
Uses & Indications
Dosage
Adults
Typical adult dosage for hypokalemia is 20-100 mEq per day, divided into 1-4 doses. For prevention, 10-25 mEq per day. Doses should be individualized based on serum potassium levels.
Elderly
Similar to adult dosage, but caution is advised due to potential for reduced renal function; monitor serum potassium closely.
Renal_impairment
Use with extreme caution; dosage reduction may be necessary. Contraindicated in severe renal impairment. Monitor serum potassium and renal function frequently.
How to Take
Take K-Nor tablets with a full glass of water, preferably after meals or with food, to minimize gastrointestinal irritation. Do not crush, chew, or suck on extended-release tablets.
Mechanism of Action
Potassium chloride acts as a replacement for endogenous potassium, maintaining intracellular osmotic pressure, aiding nerve impulse transmission, essential for cardiac electrical activity, and crucial for normal renal function and acid-base balance.
Pharmacokinetics
Onset
Effects usually observed within a few hours.
Excretion
Primarily excreted by the kidneys; a small amount is lost in stool and sweat.
Half life
Potassium homeostasis is tightly regulated, so a specific plasma half-life is not easily defined for supplemental potassium. It's rapidly excreted if levels are adequate.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract.
Metabolism
Potassium is not metabolized; it is an electrolyte.
Side Effects
Contraindications
- •Hyperkalemia (high blood potassium)
- •Severe renal impairment
- •Addison's disease (untreated)
- •Acute dehydration
- •Concomitant use of potassium-sparing diuretics in the absence of hypokalemia
Drug Interactions
NSAIDs
May reduce renal potassium excretion, potentially increasing hyperkalemia risk.
ACE inhibitors, ARBs
Increased risk of hyperkalemia.
Digitalis glycosides
Hyperkalemia can reduce the therapeutic effects of digitalis, while hypokalemia increases digitalis toxicity.
Potassium-sparing diuretics (e.g., Spironolactone, Amiloride)
Significant risk of severe hyperkalemia.
Storage
Store at room temperature (below 30°C), protected from moisture and light. Keep out of reach of children.
Overdose
Symptoms of overdose (hyperkalemia) include muscle weakness, tingling sensations, paralysis, fatigue, mental confusion, and cardiac arrhythmias (bradycardia, widened QRS, ventricular fibrillation, asystole). Management involves immediate discontinuation of potassium, IV calcium gluconate for cardiac protection, IV insulin with glucose to shift potassium intracellularly, sodium bicarbonate, and loop diuretics. Dialysis may be necessary in severe cases.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation when indicated, as potassium is a normal body constituent. However, use only if clearly needed and under medical supervision. Monitor serum potassium levels.
