Description
Overview of the medicine
Keteks (telithromycin) is a ketolide antibiotic, a subclass of macrolides, used to treat certain bacterial infections. Its use has been significantly restricted due to serious safety concerns, including liver toxicity and exacerbation of myasthenia gravis.
Uses & Indications
Dosage
Adults
60 mg IV once daily for 7-10 days, depending on infection severity and type.
Elderly
No specific dose adjustment for elderly, but monitor renal function.
Renal_impairment
For creatinine clearance <30 mL/min, reduce dose to 60 mg every other day.
How to Take
Administer as an intravenous infusion over 60 minutes. Do not administer as a bolus injection.
Mechanism of Action
Telithromycin binds to the 50S ribosomal subunit of bacteria, preventing protein synthesis. It has a high affinity for this site and can bind to two sites, leading to enhanced activity against macrolide-resistant strains.

Excretion
Mainly biliary excretion (50-60%), with renal excretion (20-30%).

Approximately 10 hours.
Absorption
Rapid and complete absorption after intravenous administration.

Side Effects
Contraindications
- •Patients with myasthenia gravis
- •History of hepatitis or jaundice associated with telithromycin or any other ketolide/macrolide antibiotic
- •Concomitant use with cisapride or pimozide
- •Hypersensitivity to telithromycin or macrolide antibiotics
Drug Interactions
Digoxin
Increased digoxin levels.
Cisapride, Pimozide
Increased risk of QT prolongation and cardiac arrhythmias (contraindicated).
Midazolam, Triazolam
Increased sedation due to CYP3A4 inhibition.
Ergot Alkaloids (e.g., Ergotamine)
Increased risk of ergot toxicity (vasospasm) (contraindicated).
Simvastatin, Lovastatin, Atorvastatin
Increased risk of rhabdomyolysis due to CYP3A4 inhibition.
Storage
Store at 20°C to 25°C (68°F to 77°F); excursions permitted to 15°C to 30°C (59°F to 86°F). Protect from light. Do not freeze.
Overdose
In case of overdose, discontinue the drug and institute symptomatic and supportive measures. Hemodialysis and peritoneal dialysis are not effective for removing telithromycin.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Telithromycin is excreted in human milk; caution should be exercised when administered to a nursing woman.
