Uses & Indications
Dosage
Adults
Oral: 200-400 mg once daily with food. Topical: Apply once or twice daily to affected area.
Elderly
No specific dose adjustment for oral, use with caution. Topical: Standard dosage.
Renal_impairment
No dose adjustment for oral, but use with caution. Topical: No adjustment needed.
How to Take
Oral tablets should be taken with food to improve absorption. Topical cream/shampoo for external use only, apply to affected area after cleaning.
Mechanism of Action
Ketoconazole inhibits the fungal cytochrome P450 enzyme 14α-demethylase, which is involved in the biosynthesis of ergosterol, a vital component of fungal cell membranes. Inhibition of ergosterol synthesis leads to increased permeability of the fungal cell membrane, resulting in leakage of cellular contents and ultimately cell death.

Clinical improvement within days to weeks depending on the infection.
Excretion
Primarily excreted in bile and feces, with a small amount excreted in urine.
Half life
Approximately 8 hours (oral).
Absorption
Variable and pH-dependent absorption when administered orally; significantly better with an acidic environment. Topical absorption is minimal.
Metabolism
Extensively metabolized in the liver via CYP3A4.

Contraindications

Drug Interactions
Antacids/H2 blockers/PPIs
Reduce oral ketoconazole absorption due to increased gastric pH.
CYP3A4 inhibitors/inducers
Ketoconazole is a potent CYP3A4 inhibitor; co-administration with other CYP3A4 substrates can increase their plasma concentrations and risk of adverse effects. Certain drugs (e.g., rifampicin, isoniazid) can reduce ketoconazole levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, abdominal pain, and liver dysfunction. Treatment is symptomatic and supportive. Gastric lavage may be considered.
Pregnancy & Lactation
