Description
Overview of the medicine
Ketotifen is a non-bronchodilator antiasthmatic agent with potent anti-anaphylactic properties and H1-antihistamine activity. It is used for the prophylactic treatment of bronchial asthma, allergic conjunctivitis, and other allergic conditions.
Uses & Indications
Dosage
Adults
Oral: 1 mg twice daily with food. If necessary, dose may be increased to 2 mg twice daily. Ophthalmic: 1 drop into the affected eye(s) twice daily.
Elderly
No specific dosage adjustment required, but use with caution due to potential age-related decrease in renal function.
Renal_impairment
Use with caution. No specific guidelines, but reduced clearance may occur. Monitor for adverse effects.
How to Take
Oral tablets/syrup should be taken with food. Eye drops should be administered topically to the affected eye(s).
Mechanism of Action
Ketotifen acts as an H1-receptor antagonist and stabilizes mast cells, thereby inhibiting the release of inflammatory mediators like histamine and leukotrienes. It also inhibits the influx of calcium into mast cells, reducing their degranulation.
Pharmacokinetics
Onset
Antihistaminic effect within 1-2 hours. Antiasthmatic prophylactic effect may take several weeks (typically 2-12 weeks).
Excretion
Approximately 60-70% of a dose is excreted renally, mainly as metabolites, within 48 hours.
Half life
Biphasic elimination; terminal half-life is approximately 21 hours for oral administration.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Bioavailability is approximately 50% due to first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily by glucuronidation. Major metabolite is ketotifen-N-glucuronide.
Side Effects
Contraindications
- Hypersensitivity to ketotifen or any component of the formulation
- Acute asthma attacks (not indicated for acute relief)
- Do not discontinue existing antiasthmatic therapy abruptly upon initiation of ketotifen
Drug Interactions
Oral antidiabetics
Reversible fall in platelet count has been observed in a few patients.
CNS depressants (e.g., hypnotics, sedatives, alcohol)
May potentiate the effects of ketotifen.
Storage
Store below 25-30°C, in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, confusion, disorientation, tachycardia, and possibly convulsions (especially in children). Treatment is symptomatic and supportive, including gastric lavage and activated charcoal if recent ingestion.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding, as ketotifen is excreted in human milk.
