Overview of the medicine
Ketorolac (marketed as Ketovir) is a nonsteroidal anti-inflammatory drug (NSAID) used for the short-term treatment of moderately severe acute pain that requires analgesia at the opioid level. It is available in oral, injectable, and ophthalmic forms.
Uses & Indications
Dosage
Adults
Oral: 10 mg every 4-6 hours as needed, not to exceed 40 mg/day. Max duration 5 days.
Elderly
Lower doses may be required, e.g., 10 mg every 6-8 hours, max 30 mg/day.
Renal_impairment
Reduced dosage is necessary, often contraindicated in severe impairment.
How to Take
Oral tablets should be taken with food or milk to reduce gastrointestinal irritation. Injection is for IM or IV use.
Mechanism of Action
Ketorolac inhibits prostaglandin synthesis by reversibly blocking the cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes. This leads to a reduction in inflammation, pain, and fever.

Onset
Within 1 hour for oral, 30 minutes for IM.
Excretion
Primarily renal (approximately 90%), some through feces.
Half life
Approximately 4-6 hours (can be longer in elderly or renal impairment).
Absorption
Rapid and complete after oral administration. Peak plasma concentrations reached within 30-60 minutes.
Metabolism
Primarily hepatic (glucuronidation, hydroxylation).

Drug Interactions
Lithium

Reduced antihypertensive effect and increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include abdominal pain, nausea, vomiting, and drowsiness. Management involves symptomatic and supportive care, including gastric lavage or activated charcoal if recent ingestion.
Pregnancy & Lactation
Contraindicated in the third trimester of pregnancy due to risk of premature closure of fetal ductus arteriosus. Not recommended during breastfeeding as it is excreted in breast milk.
