Kfore 125 mg Suspension is an antibiotic medication containing Cefixime, a third-generation cephalosporin. It is used to treat a wide range of bacterial infections in children, including those of the ear, nose, throat, chest, urinary tract, and skin.
Uses & Indications
Dosage
Adults
Not typically used for adults in 125 mg suspension form; adult dosage for Cefixime is usually 200-400 mg daily.
Elderly
Not typically used for elderly in 125 mg suspension form; dosage adjustment may be needed based on renal function if higher doses are used.
Renal_impairment
For children with creatinine clearance 21-60 ml/min, 7.5 mg/kg daily; for creatinine clearance < 20 ml/min, 3.75 mg/kg daily. For adults, dose adjustment needed.
How to Take
Shake the bottle well before each use. Administer orally, with or without food. Complete the full course of treatment as prescribed by the doctor, even if symptoms improve.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby interfering with the transpeptidation process that cross-links peptidoglycan strands, which are essential for bacterial cell wall integrity and rigidity. This leads to impaired cell wall formation, resulting in bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-6 hours after administration.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine over 24 hours. The remainder is excreted via non-renal mechanisms, primarily through bile and feces.
Half life
The plasma half-life is typically 3-4 hours.
Absorption
Approximately 40-50% of an oral dose is absorbed from the gastrointestinal tract. Food delays the time to peak concentration but does not significantly alter the extent of absorption.
Metabolism
Cefixime is not extensively metabolized in the body. No active metabolites have been identified.

Contraindications
- Known hypersensitivity to Cefixime or other cephalosporins.
- History of serious hypersensitivity reactions to penicillin.
Drug Interactions
Nifedipine
Concurrent use may increase Cefixime bioavailability.
Carbamazepine
Increased carbamazepine levels have been reported when administered concomitantly with Cefixime.
Warfarin and other anticoagulants
May increase prothrombin time (PT) with or without bleeding. Close monitoring of PT/INR is recommended.
Storage
Store dry powder at room temperature below 30°C, protected from light and moisture. Store reconstituted suspension in a refrigerator (2-8°C). Do not freeze.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Management should be supportive.
