Description
Overview of the medicine
Kilbac 15 gm Injection contains Human Normal Immunoglobulin, a sterile preparation of highly purified immunoglobulin G (IgG) antibodies derived from pooled human plasma. It is used to provide passive immunity and modulate the immune system in various conditions.
Uses & Indications
Dosage
Adults
Dosage varies significantly by indication. For primary immunodeficiency: 0.4-1 g/kg every 3-4 weeks. For ITP: 0.4 g/kg daily for 2-5 days or 1 g/kg for 1-2 days. Max infusion rate depends on patient tolerance and product guidelines.
Elderly
Generally, no specific dose adjustment is required, but caution should be exercised, especially in patients with pre-existing renal or cardiovascular risk factors.
Renal_impairment
Use with caution and monitor renal function. Dosage adjustment may be necessary in patients at risk of acute renal failure.
How to Take
For intravenous infusion only. Administer at a slow initial rate, gradually increasing if tolerated. Do not shake the vial. Inspect visually for particulate matter and discoloration prior to administration.
Mechanism of Action
Human Normal Immunoglobulin provides a broad spectrum of IgG antibodies against various infectious agents. It also exerts immunomodulatory effects by blocking Fc receptors, altering cytokine production, and modulating B-cell and T-cell function.
Pharmacokinetics
Onset
Immediate.
Excretion
Degraded via normal protein catabolic pathways.
Half life
Approximately 21 to 28 days, though it can vary depending on the individual and the underlying condition.
Absorption
Administered intravenously, resulting in 100% systemic bioavailability.
Metabolism
IgG molecules are catabolized in the same way as endogenous IgG, through the reticuloendothelial system.
Side Effects
Contraindications
- Known hypersensitivity to human immunoglobulins or any excipients.
- IgA deficient patients with known antibodies against IgA, due to risk of anaphylaxis.
Drug Interactions
Loop Diuretics
May increase the risk of renal dysfunction in combination with certain IVIg products.
Live attenuated virus vaccines
May impair the efficacy of live attenuated virus vaccines (e.g., measles, mumps, rubella, varicella) for up to 3 months. Vaccination should be postponed for at least 3 months after IVIg administration.
Storage
Store at 2°C to 8°C (36°F to 46°F). Do not freeze. Protect from light. Once reconstituted or opened, use immediately.
Overdose
Overdose is unlikely given the intravenous route and close monitoring during administration. However, volume overload and hyperviscosity could occur. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation. IgG antibodies are known to cross the placental barrier and are excreted in breast milk, but no adverse effects on the fetus or breastfed infant are expected.

Lab Monitoring
- Monitor renal function (creatinine, BUN) before and during treatment, especially in at-risk patients.
- Monitor serum osmolality and urine output.
- Check blood pressure and vital signs during infusion.
- In patients with suspected hemolytic anemia, monitor hemoglobin and haptoglobin levels.
Doctor Notes
- Always administer at the lowest effective dose and slowest infusion rate tolerable.
- Monitor patients for signs of adverse reactions, especially during the first hour of infusion.
- Ensure adequate hydration prior to infusion to minimize renal adverse events.
Patient Guidelines
- Report any adverse reactions immediately.
- Stay well-hydrated before and during infusion.
- Do not take any live attenuated vaccines without consulting your doctor.
Missed Dose Advice
If a dose is missed, contact your doctor immediately for advice on rescheduling the infusion. Do not double the dose.
