Description
Overview of the medicine
Kilbac 250 mg Injection contains Clindamycin, a lincosamide antibiotic used to treat a wide variety of bacterial infections. It works by stopping the growth of bacteria. It is effective against certain anaerobic bacteria and some Gram-positive aerobic bacteria.
Uses & Indications
Dosage
Adults
For serious infections: 600-1200 mg/day in 2-4 equally divided doses. For more severe infections: 1200-2700 mg/day in 2-4 equally divided doses. Administer IM or IV.
Elderly
Dosage adjustment not generally required based on age alone, but monitor renal and hepatic function.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe impairment (CrCl <30 mL/min) or end-stage renal disease, consider dosage reduction or increased dosing interval.
How to Take
Kilbac 250 mg Injection is administered intramuscularly (IM) or intravenously (IV). For IV infusion, it must be diluted prior to administration and infused over at least 10-60 minutes depending on the dose. Do not administer IV bolus.
Mechanism of Action
Clindamycin binds to the 50S ribosomal subunit of susceptible bacteria, thereby inhibiting protein synthesis. It is primarily bacteriostatic but can be bactericidal at higher concentrations or against highly susceptible organisms.
Pharmacokinetics
Onset
Varies depending on infection type and severity; generally within hours for antibacterial effect.
Excretion
Excreted in urine (approx. 10%) and feces (approx. 4%) as active drug and metabolites.
Half life
Approximately 2-3 hours in adults with normal renal function.
Absorption
Rapidly absorbed after intramuscular administration. Peak serum concentrations are reached within 1-3 hours after IM injection.
Metabolism
Primarily metabolized in the liver to active and inactive metabolites (e.g., N-demethyl clindamycin and clindamycin sulfoxide).
Side Effects
Contraindications
- Hypersensitivity to Clindamycin, Lincomycin, or any component of the formulation
- History of antibiotic-associated colitis
Drug Interactions
Erythromycin
Antagonism may occur due to competition for the 50S ribosomal binding site.
Kaolin-pectin
May reduce the absorption of oral clindamycin (not relevant for injection, but good to note for the drug).
Neuromuscular blocking agents
May enhance the neuromuscular blocking action, leading to increased muscle weakness.
Storage
Store at controlled room temperature (20°C to 25°C), protect from freezing. Do not refrigerate diluted solutions.
Overdose
No specific antidote. Manage overdose with symptomatic and supportive care. Hemodialysis and peritoneal dialysis are not effective in removing clindamycin from the blood.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Clindamycin is excreted in breast milk; use with caution in nursing mothers.

Lab Monitoring
- Regular monitoring of liver and kidney function (especially in prolonged therapy or in neonates/infants)
- Blood counts
- Monitoring for signs of C. difficile-associated diarrhea
Doctor Notes
- Assess patient's history for colitis or severe diarrhea before initiating therapy.
- Adjust dosage in severe hepatic impairment, although not typically required for renal impairment.
- Administer IV slowly to avoid adverse cardiac effects and ensure proper dilution.
Patient Guidelines
- Report any severe or persistent diarrhea to your doctor immediately.
- Inform your doctor about all other medications you are taking.
- Complete the full course of treatment as prescribed, even if you feel better.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
No specific precautions; clindamycin is not known to impair the ability to drive or operate machinery.
Lifestyle Advice
- Maintain good hygiene to prevent secondary infections.
- Stay hydrated, especially if experiencing diarrhea.
