Description
Overview of the medicine
Koftex syrup is used to relieve cough associated with colds, bronchitis, and other respiratory tract infections. It helps to suppress cough and loosen mucus, making it easier to expel.
Uses & Indications
Dosage
Adults
Adults and children over 12 years: 10 ml (2 teaspoonfuls) every 4-6 hours, not exceeding 60 ml (12 teaspoonfuls) in 24 hours.
Elderly
No specific dose adjustment is generally required, but caution is advised in elderly patients with impaired renal or hepatic function.
Renal_impairment
Use with caution in patients with severe renal impairment; dose adjustment may be necessary.
How to Take
For oral use only. Shake the bottle well before use. May be taken with or without food.
Mechanism of Action
Dextromethorphan acts centrally on the cough center in the medulla to elevate the cough threshold. Guaifenesin works by increasing the volume and decreasing the viscosity of secretions in the trachea and bronchi, thereby facilitating the removal of secretions by ciliary action and cough.

Onset
Dextromethorphan: 15-30 minutes; Guaifenesin: 30 minutes
Excretion
Primarily excreted in the urine as unchanged drug and metabolites.
Half life
Dextromethorphan: 1.4-3.9 hours; Guaifenesin: 1 hour
Absorption
Dextromethorphan is rapidly absorbed from the gastrointestinal tract. Guaifenesin is also well absorbed orally.
Metabolism
Dextromethorphan is extensively metabolized in the liver by CYP2D6. Guaifenesin is metabolized rapidly in the liver.
Side Effects
Contraindications
- •Hypersensitivity to Dextromethorphan, Guaifenesin, or any component of the formulation.
- •Patients receiving monoamine oxidase inhibitors (MAOIs) or within 14 days of discontinuing MAOI therapy.
Drug Interactions
MAOIs
Concomitant use with MAOIs can lead to a risk of serotonin syndrome.
CNS Depressants
May enhance the sedative effects of alcohol, antihistamines, psychotropics, and other CNS depressants.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, dizziness, drowsiness, ataxia, and respiratory depression (with very high doses of Dextromethorphan). Management is symptomatic and supportive. Gastric lavage or activated charcoal may be considered if taken recently.
Pregnancy & Lactation
Use during pregnancy and lactation only if clearly needed and after consulting a doctor. There is limited data on its safety in these populations.
