Overview of the medicine
Krimizole DS is a broad-spectrum anthelmintic medication containing Mebendazole. It is used to treat single or mixed infestations of intestinal nematodes like pinworm, roundworm, whipworm, and hookworm. The DS (Double Strength) formulation provides a higher dose for effective single-dose treatment.
Uses & Indications
Dosage
Adults
For most intestinal worm infestations, a single dose of 500 mg is usually sufficient. In case of re-infestation, the treatment may be repeated after 2-4 weeks. For pinworm, treating all household members simultaneously is recommended.
Elderly
No specific dosage adjustment is generally required for elderly patients, but caution is advised in those with hepatic impairment.
Renal_impairment
No specific dosage adjustment is required in renal impairment as it is poorly absorbed and primarily eliminated via faeces.
How to Take
The tablet can be chewed, swallowed whole, or crushed and mixed with food. It can be taken with or without food, but absorption is increased with fatty meals.
Mechanism of Action
Mebendazole selectively and irreversibly blocks glucose uptake and other nutrients in susceptible intestinal helminths. This leads to glycogen depletion, reduced ATP production, and eventually the death of the helminth.

Onset
Effects begin within hours, complete worm expulsion may take several days.
Excretion
Mainly excreted in faeces as unchanged drug or metabolites. A small portion is excreted in urine.
Half life
Approximately 2.5-5.5 hours.
Absorption
Poorly absorbed from the gastrointestinal tract (5-10%). Absorption is increased with fatty meals.
Metabolism
Extensively metabolized in the liver, primarily to its hydroxylated metabolite.

Contraindications
- Hypersensitivity to Mebendazole or any component of the formulation
- Pregnancy (especially first trimester)
Drug Interactions
Cimetidine
May inhibit the metabolism of Mebendazole, leading to increased plasma concentrations.
Metronidazole
Concurrent use of Mebendazole and Metronidazole has been associated with Stevens-Johnson syndrome/toxic epidermal necrolysis.
Phenytoin/Carbamazepine
May decrease Mebendazole plasma concentrations due to enzyme induction.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

In cases of accidental overdose, gastrointestinal symptoms (abdominal cramps, nausea, vomiting, diarrhea) may occur. Treatment should be symptomatic and supportive. Activated charcoal may be considered to reduce absorption if taken recently.
Pregnancy & Lactation
Pregnancy Category C. Mebendazole should not be used in pregnant women, especially during the first trimester, unless the potential benefit justifies the potential risk to the fetus. It is unknown whether mebendazole is excreted in human milk. Caution should be exercised when administered to a nursing woman.
