Overview of the medicine
Lamotrigine is an anticonvulsant medication used to treat epilepsy (seizures) and bipolar disorder. It helps to stabilize electrical activity in the brain.
Uses & Indications
Dosage
Adults
Varies significantly based on indication and concomitant medications. Typically starting with 25 mg daily, titrated slowly upwards over several weeks to a usual maintenance dose of 100-400 mg/day.
Elderly
No specific dosage adjustments required unless hepatic or renal impairment is present. Start at lower end of dosing range.
Renal_impairment
Reduced maintenance dosage may be required. Caution advised. Consider using a lower initial dose and slower escalation.
How to Take
Oral administration, with or without food. Chewable/dispersible tablets can be swallowed whole, chewed, or dispersed in a small amount of water.

Side Effects
Contraindications

Drug Interactions
Valproate
Significantly increases lamotrigine half-life and plasma levels, requiring dose reduction of lamotrigine.
Rifampicin
Decreases lamotrigine plasma levels.
Oral contraceptives (estrogen-containing)
May decrease lamotrigine levels, leading to loss of seizure control. Requires monitoring and dose adjustment.
Carbamazepine, Phenytoin, Phenobarbital, Primidone
Decrease lamotrigine plasma levels, requiring dose increase of lamotrigine.
Storage

Overdose
Symptoms include nystagmus, ataxia, dizziness, headache, drowsiness, seizures, and coma. No specific antidote; symptomatic and supportive treatment. Gastric lavage and activated charcoal may be considered.

Pregnancy Category C. Use only if potential benefit justifies potential risk. Higher doses may be needed during pregnancy. Excreted in breast milk, use with caution, monitor infants for adverse effects.
