Description
Overview of the medicine
Lamitrin (Lamotrigine) is an anticonvulsant medication used to treat epilepsy and bipolar disorder. It helps to stabilize electrical activity in the brain and prevent seizures or extreme mood swings.
Uses & Indications
Dosage
Adults
Initial dosage typically 25 mg once daily, slowly titrated upwards over several weeks based on response and tolerability. Maintenance dose can range from 100-400 mg/day in divided doses. 2 mg is usually a very initial or specific starting dose for very sensitive patients or specific titration schedules.
Elderly
Similar to adult dosing, but with careful monitoring for adverse effects.
Renal_impairment
Reduced maintenance doses should be considered; careful monitoring is essential.
How to Take
Take orally, with or without food. Swallow tablets whole; do not chew or crush. Dosage should be titrated slowly as directed by a physician.
Mechanism of Action
Lamotrigine stabilizes neuronal membranes and inhibits the release of excitatory neurotransmitters (primarily glutamate and aspartate) by blocking voltage-sensitive sodium channels.
Pharmacokinetics
Onset
Effects may take several weeks to become apparent due to slow titration.
Excretion
Mainly renal excretion of metabolites and unchanged drug.
Half life
Approximately 25-33 hours in adults (can be affected by concomitant medications).
Absorption
Rapid and complete absorption from the gastrointestinal tract.
Metabolism
Primarily hepatic glucuronidation (via UGT enzymes).
Side Effects
Contraindications
- •Hypersensitivity to lamotrigine or any component of the formulation
Drug Interactions
Valproate
Increases lamotrigine plasma levels by inhibiting its metabolism, increasing risk of side effects, especially rash.
Oral Contraceptives
May decrease lamotrigine levels, requiring dosage adjustment; lamotrigine may also reduce contraceptive efficacy.
Carbamazepine, Phenytoin, Phenobarbital, Primidone
Decreases lamotrigine plasma levels by inducing its metabolism, potentially reducing efficacy.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nystagmus, ataxia, dizziness, somnolence, headache, and severe skin rash. Management is supportive, including gastric lavage or activated charcoal if recent ingestion, and close monitoring of vital signs. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Lamotrigine is excreted into breast milk; caution should be exercised when administered to a nursing mother. Consult a doctor.
