Lamotrigine is an anticonvulsant drug primarily used to treat epilepsy and bipolar disorder. It helps to stabilize electrical activity in the brain.
Uses & Indications
Dosage
Adults
Initial dose typically 25 mg once daily for two weeks, then 50 mg once daily for two weeks. Dosage is then gradually increased to a maintenance dose, usually 100-400 mg/day, depending on concomitant medications.
Elderly
Similar to adults, but dosage adjustments may be necessary based on renal or hepatic function and careful monitoring.
Renal_impairment
Reduced maintenance doses may be required in patients with significant renal impairment. Use with caution and monitor closely.
How to Take
Take orally, with or without food. Tablets can be swallowed whole, chewed, or dispersed in water.
Mechanism of Action
Lamotrigine works by blocking voltage-sensitive sodium channels in neuronal membranes, which stabilizes the membranes and inhibits the release of excitatory neurotransmitters (primarily glutamate and aspartate).

Side Effects
Contraindications

Drug Interactions
Valproate
Increases lamotrigine plasma concentrations by inhibiting its metabolism, requiring a reduced lamotrigine dose.
Rifampicin
Decreases lamotrigine plasma concentrations.
Oral Contraceptives (estrogen-containing)
May decrease lamotrigine plasma concentrations, requiring dose adjustments.
Carbamazepine, Phenytoin, Phenobarbital, Primidone
Decrease lamotrigine plasma concentrations by inducing its metabolism, requiring an increased lamotrigine dose.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include ataxia, nystagmus, increased seizures, decreased consciousness, coma, and rarely, death. Management is supportive; activated charcoal may be considered if taken recently.

Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Lamotrigine is excreted into breast milk; monitor infants for adverse effects. Consult a doctor.
