Overview of the medicine
Lamotrigine is an antiepileptic drug used to treat seizures in epilepsy and to delay mood episodes in bipolar disorder. It works by stabilizing electrical activity in the brain.
Uses & Indications
Dosage
Adults
Initial dosage typically 25 mg once daily for two weeks, followed by 50 mg once daily for two weeks. Dose is then gradually increased to a maintenance range, typically 100-400 mg/day in divided doses, depending on indication and concomitant medications. For bipolar disorder, usual maintenance is 100-400 mg/day.
Elderly
Similar to adult dosage; however, close monitoring is advised due to potential age-related organ impairment and polypharmacy.
Renal_impairment
Reduced maintenance dose may be necessary. For severe renal impairment, lower initial and maintenance doses should be considered.
How to Take
Oral administration. Tablets can be taken with or without food. Chewable/dispersible tablets can be chewed or dissolved in water.
Mechanism of Action
Lamotrigine works by blocking voltage-sensitive sodium channels, thereby stabilizing neuronal membranes and inhibiting the release of excitatory neurotransmitters like glutamate and aspartate.

Contraindications

Drug Interactions
Valproate
Increases lamotrigine levels by inhibiting its metabolism, necessitating a significant reduction in lamotrigine dosage.
Oral Contraceptives
Can decrease lamotrigine levels, requiring dose adjustments.
Carbamazepine, Phenytoin, Phenobarbital, Primidone
Decrease lamotrigine levels by inducing its metabolism, potentially requiring higher lamotrigine doses.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nystagmus, ataxia, impaired consciousness, grand mal seizures, and coma. Management is supportive, including gastric lavage, activated charcoal, and close monitoring of vital signs. No specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Lamotrigine is excreted in breast milk; caution should be exercised when administered to a nursing woman. Discuss risks and benefits with your doctor.
