Description
Overview of the medicine
Levofloxacin is a broad-spectrum fluoroquinolone antibiotic used to treat various bacterial infections including pneumonia, bronchitis, sinusitis, skin and soft tissue infections, and urinary tract infections.
Uses & Indications
Dosage
Adults
Typically 250 mg to 500 mg orally once daily, depending on the type and severity of infection. For some severe infections, 750 mg once daily might be prescribed.
Elderly
Similar to adults, but careful monitoring of renal function is advised, and dose adjustments may be necessary.
Renal_impairment
Dose adjustment is required based on creatinine clearance. For CrCl 20-49 mL/min, 250 mg every 48 hours; for CrCl 10-19 mL/min, 250 mg initial dose then 125 mg every 48 hours.
How to Take
Levomax TS tablets can be taken orally with or without food. It should be swallowed whole with a sufficient amount of liquid. Do not crush or chew.
Mechanism of Action
Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.

Onset of action within 1-2 hours of administration.
Excretion
Primarily excreted unchanged in the urine (approximately 87% within 48 hours).
Half life
Elimination half-life is typically 6-8 hours.
Absorption
Rapid and nearly complete oral absorption; peak plasma concentrations achieved within 1-2 hours. Bioavailability is approximately 99%.
Metabolism
Minimally metabolized in the liver; less than 5% is metabolized.

Contraindications
- Hypersensitivity to levofloxacin or other fluoroquinolones
- History of tendinitis or tendon rupture associated with fluoroquinolone use
- Epilepsy
- Children and adolescents during growth period
- Pregnancy and breastfeeding
Drug Interactions

Antacids (containing magnesium or aluminum) and sucralfate: Can reduce levofloxacin absorption. Administer levofloxacin at least 2 hours before or 2 hours after these agents.

Iron and Zinc supplements: Can reduce absorption. Administer levofloxacin at least 2 hours before or 2 hours after these supplements.

NSAIDs: May increase the risk of CNS stimulation and convulsive seizures.

Warfarin: May enhance the anticoagulant effects of warfarin, requiring monitoring of INR.

Corticosteroids: Increased risk of tendinitis and tendon rupture.

Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.

In the event of an acute overdose, the stomach should be emptied. The patient should be observed and hydration maintained. ECG monitoring is advised due to the potential for QT prolongation. Hemodialysis or peritoneal dialysis is not effective in removing levofloxacin.

Pregnancy Category C. Levofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, breastfeeding is not recommended during treatment.
