Uses & Indications
Dosage
Adults
750 mg orally once daily for 5-14 days, depending on the type and severity of infection.
Elderly
No specific dose adjustment required based on age, but renal function should be assessed.
Renal_impairment
Dose adjustment necessary for creatinine clearance < 50 mL/min. For CrCl 20-49 mL/min: 750 mg every 48 hours. For CrCl 10-19 mL/min: Initial 750 mg, then 500 mg every 48 hours.
How to Take
Swallow the tablet whole with water, with or without food. Do not crush, cut, or chew. Ensure adequate hydration.
Mechanism of Action
Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.

Onset
Within 1-2 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine (approximately 87% of dose within 48 hours).
Half life
Approximately 6-8 hours.
Absorption
Rapid and almost complete absorption after oral administration; peak plasma concentrations reached within 1-2 hours.
Metabolism
Minimally metabolized in the liver (less than 5% of dose).
Side Effects
Contraindications
- Hypersensitivity to levofloxacin or other fluoroquinolones
- History of tendon disorders related to fluoroquinolone administration
- Epilepsy or other seizure disorders
- Children and adolescents (under 18 years of age)
- Pregnancy and lactation (unless benefits outweigh risks)
Drug Interactions
NSAIDs
May increase the risk of CNS stimulation and convulsive seizures.
Warfarin
May enhance anticoagulant effects, leading to increased bleeding risk. Monitor INR.
Corticosteroids
Increased risk of tendon rupture when co-administered with fluoroquinolones.
Antiarrhythmics (Class IA and III)
Increased risk of QT prolongation. Use with caution.
Antacids (magnesium/aluminum), sucralfate, iron/zinc supplements
Reduce levofloxacin absorption. Administer levofloxacin at least 2 hours before or 2 hours after these agents.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include CNS effects (confusion, dizziness, seizures), prolonged QT interval, nausea, and vomiting. Management involves symptomatic and supportive care, including gastric lavage. Hemodialysis is not effective in removing levofloxacin.

Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as levofloxacin is excreted in human milk.
