Overview of the medicine
LG-M is a combination medication containing Levocetirizine, an antihistamine, and Montelukast, a leukotriene receptor antagonist. It is primarily used to relieve symptoms of allergic rhinitis (seasonal and perennial) and for the prophylactic treatment of chronic asthma.
Uses & Indications
Dosage
Adults
One tablet (Levocetirizine 5 mg + Montelukast 10 mg) once daily, preferably in the evening.
Elderly
No dose adjustment is generally required in elderly patients, unless there is renal impairment.

Dose adjustment for Levocetirizine may be necessary based on creatinine clearance. Consult a physician.
How to Take
Take orally with or without food, preferably in the evening.
Mechanism of Action
Levocetirizine selectively antagonizes H1-receptors, thereby inhibiting the effects of histamine. Montelukast selectively antagonizes the cysteinyl leukotriene type 1 (CysLT1) receptor, blocking the action of leukotrienes, which are inflammatory mediators involved in asthma and allergic rhinitis.

Levocetirizine: ~1 hour. Montelukast: Clinical improvement may be seen within 1 day, full effect over several days.
Excretion
Levocetirizine is primarily excreted unchanged via urine. Montelukast is primarily excreted in the feces via bile.
Half life
Levocetirizine: ~7-10 hours. Montelukast: ~2.7-5.5 hours.
Absorption
Levocetirizine is rapidly and extensively absorbed after oral administration, reaching peak plasma concentrations in about 1 hour. Montelukast is rapidly absorbed, with peak plasma concentrations occurring 2-4 hours after oral administration.
Metabolism
Levocetirizine undergoes minimal metabolism. Montelukast is extensively metabolized in the liver by CYP3A4, 2C8, and 2C9.
Side Effects
Contraindications
- Hypersensitivity to levocetirizine, montelukast, or any component of the formulation.
- Severe renal impairment (creatinine clearance <10 ml/min).
- Severe hepatic impairment.
Drug Interactions
Rifampin
May decrease the plasma concentration of montelukast.
Phenobarbital
May decrease the bioavailability of montelukast.
CNS depressants
Concomitant use with levocetirizine may increase CNS depression (e.g., alcohol, sedatives).
